No violations noted on the previous inspection conducted on 10/09/15.

with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and
any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

AREAS OF CONCERN AND COMMENTS:

1) Required signage present. Need to clear the vegetation from around the signs so that they are easily visible.
2) Access road is in good shape.
3) Markers either down or missing along the southern and western sides of the field. See map below.
4) No septage discharges noticed outside of the permitted boundary of the field.
5) No application issues noticed today. Septage had been applied to the eastern side of the field.
6) Good stand of bermudagrass. Some weeds present.
7) No application at time of inspection. pH not checked.
8) No objectionable odors noticed today.
9) Setback distances not checked.
10) Application for permit renewal received on 08/03/15.

Missing or down markers at the stars on the map. Good bermudagrass coverage in the field.
