The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Field 2 has a significant amount of weeds & vegetation other than fescue. This was previously noted on
the last audit of 11/15/2017. Field 2 needs cut (per the 30 day wait after the last discharge event) and
weed control needs to be implemented as soon as possible. Discussed the cropstand issues with Clay Oaks
on my way out of the site.
2. Land application has occurred on Field 2 (which is permitted as active). Field 1 is inactive per the current
permit & does not have an established fescue cropstand at this time. No land application shall occur on Field 1.
3. No sign of heavy application or application outside the permitted boundaries. It appears that the approved
setbacks are being met.
4. Field markers appear adequate except markers are missing on the back end of Field 2 (south end) to
delineate the disposal field boundary in regard to the adjacent field.
5. The required signs are posted and the access road is adequate.
6. Soil samples shall be taken by the permit holder this fall. One box for active Field 2.
