No violations noted on the previous inspection conducted on 01/20/17.

OBSERVED VIOLATIONS:

with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and
any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

AREAS OF CONCERN AND COMMENTS:

1) Signage present on both sides of the path as you get to the site. Signs need to be repainted.
2) Access road is well maintained.
3) Disposal boundaries for both fields clearly marked. Markers visible.
4) All septage discharges within the permitted boundaries of the fields.
5) No spot dumps or thick applications noticed today. Septage had been applied to Field A prior to my arrival. No
ponding observed in the field.
6) Some broadleaf weed control needed in both fields.
7) No application at time of inspection. pH not checked.
8) No objectionable odors noticed today.
9) New landowner for the property to the southern edges of Fields A and B. Property line had been resurveyed
and marked. Current markers along the southern edge to Field A are a little to close to the property line. I
measured off the 50 feet setback requirement to the property line and placed orange flags as to where the
markers need to be moved.
10) Application for permit renewal received on 09/09/15. Application under review.
