with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and
any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

AREAS OF CONCERN AND COMMENTS:

1) Signage present on both sides of the path as you get to the site. Signs have been repainted. Thank you.
2) Access road is well maintained.
3) Disposal boundaries for both fields clearly marked.
4) No septage discharges noticed outside of the permitted boundary.
5) No spot dumps or thick applications noticed today.
6) Some broadleaf weed control needed in both fields.
7) No application at time of inspection. pH not checked.
8) No objectionable odors noticed today.
9) Markers have not been moved along the southern edge to Field A. Orange flags still present. No application of
septage outside of the orange flags. Markers need to be moved to where the orange flags are.
10) Application for permit renewal received on 09/09/15. Application under review.
