The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and
any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

AREAS OF CONCERN AND COMMENTS:

1) Inspected pump truck with license plate # 46560P. All required information listed on both sides of the truck except
for town name and state. While “Linville Division” is listed underneath the firm name, the truck is based in
Virginia and not North Carolina. Also, there is a Linville, North Carolina. Virginia can be added within “Linville
Division” to read as “Linville VA Division” on both sides of the truck. The other way to handle this issue would
be to add “Linville VA” to both sides of the truck. As with the other lettering, make sure that each letter is at least
3 inches in height. Talked to Mr. Grizzard on 01/23/17 about the truck labeling.

2) Tank is made of metal and appears to be structurally sound.
3) Tank is bolted to the frame of the truck.
4) All valves appeared to be in working order. Cap present on uptake and discharge ports. Other access ports were
closed and appeared to have proper fitting lids.
5) No leaks noticed.
6) New firm. Truck logs not checked.
7) Pictures received by email from Mr. Grizzard on 01/29/17 showing where Virginia, abbreviated as “VA,” had been
included within “Linville Division” on both sides of the truck. Thus, it reads as “Linville, VA Division” on both
sides of the truck. See pictures below. This will suffice for town name and state. The modified lettering appears
to be as tall as the lettering above and below which was greater than 3 inches.
8) With the pictures received on 01/29/17, pump truck with license plate # 46560P is approved for hauling of
grease septage in NC. The tankers listed on the application were not inspected at this time. The tankers
cannot be used in NC to haul grease septage until they have been inspected and approved by the Division.
Please contact me when the tankers are ready to be inspected.
