or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and
any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

1) Inspected pump truck with VIN # 1NPCXPEX4JD454794 on 6/13/17. Inspected pump truck with VIN #
1NPCXPEX4JD45725 on 6/14/17. All required information listed on both sides of each pump truck. All
required letters were at least 3 inches in height. The phone number listed on the pump trucks is 252-
489-3366. See pictures below. Both trucks are new. Received the license plate number for the second
truck on 6/23/17.

2) Tanks on both pump trucks are made of steel and appear to be structurally sound.
3) The tanks for both pump trucks are bolted to the frame of the trucks.
4) All valves appeared to be in working order. Cap present on discharge port. Top access ports had
proper fitting lids.
5) No leaks noticed.
6) New firm. Truck logs not checked.
7) Both trucks passed inspection.
