Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 2 AREAS OF CONCERN AND COMMENTS: TRUCK LICENSE PLATE # VIN # TANK CAPACITY (gal) YA136320 1FVACXDC66HV52562 2,500 1) Inspected pump truck with license plate # YA136320. All required information listed on both sides of the truck and all lettering was at least 3 ¼ inches in height. The town was listed on the truck’s doors. The other required information was listed either on the tank or fender around tank. Inc. had been listed with Pipeworx Plumbing on the application for Firm Name. Talked to Weston and he said that Inc. was not on their plumbing trucks. Suggested taking it off the application. Driver side of the truck on 01/23/17. Passenger side of the truck on 01/23/17. 2) Tank is made of metal and appears to be structurally sound. Tank had been repainted. 3) Tank is bolted to the frame of the truck. 4) All valves appeared to be in working order. Cap present on uptake and discharge ports. Other access ports were closed and appeared to have proper fitting lids. 5) No leaks noticed. 6) New firm. Truck logs not checked. 7) Truck passed inspection.
