The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Access to the landfill is located on the SW corner controlled by a locked gate.
2. Waste in the landfill consisted of fiberglass generated from PPG Industries Inc.
3. Mr. Gurley stated the landfill was in operation for ten years.
4. The landfill had well established vegetative cover and is free of trees and woody vegetation.
5. No leachate seeps or erosion of the cap were observed.
6. Observed edge of waste markers.

7. Republic Services contracts Side Seeding twice a year to bush hog the landfill. Landfill is typically bush hogged
April-May and September-October. Side Seeding was on-site 5-21-18 and bush hogged the entire landfill.
8. Mr. Gurley confirmed Side Seeding would to be on-site today (5-22-18) to weed eat around all monitoring wells
and seed and sow a few areas where repairs were made on berms.
9. 1st annual groundwater monitoring report for 2017 reviewed.
10. Methane monitoring was being performed as required. Reviewed 1st quarter 2017, 2nd quarter 2017, 3rd quarter
2017, 4th quarter 2017 and 1st quarter 2018.
11. A random inspection of three monitoring wells was performed to verify proper tags and locks.
