FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 2 ADDITIONAL COMMENTS 1. Once a year on site meeting. Discussed past audits & site conditions. Reviewed submittal for permit renewal – made minor revisions. Per the submitted logs the site has not been used a lot over the past year. Soil samples will be taken by the permit holder this fall. 2. The cropstand appears adequate at this time. Hay bales are currently stored along the field perimeter, need to be moved in the near future. 3. Received key for new gated entrance. If lock changes in the future please let me know. 4. Site is managed as one field. It appears that the approved setbacks are being met but were not measured at this time. Noted that the field markers along the 50’property line on the northwest side need to be straightened in agreement with the 50' setback. The corner field marker on the south west side is partly hidden by vegetation which needs removed. Maintain all required setbacks. 5. No sign of land application outside the permitted boundaries. No heavy or recent application. No odors noted. Waste is applied by irrigation. 6. The required signs are posted and the access road is adequate. 7. Submittal is under review at this time for permit renewal. 8. Thank you for your time.
