The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Proper signage was observed at the entrance of the facility.
2. The facility is permitted to receive solid waste within approximately 100 miles around the landfill and solid waste
generated within Burke County.
3. The facility accepts an average of 150 tons per day.
4. To minimize the number of small vehicles on the working face, a convenience center area is set up with a roll off
for the public to utilize for smaller loads.
5. Edge of waste markers were visible.
6. All access roads were maintained in good condition.
7. Working face was small and compact.
8. Two tires were observed near the SW slope of the working face and removed by facility staff. No other
unacceptable waste was observed.
9. A “roll off container” is staged near the working face for the collection of scrap tires.
10. The facility utilizes soil for required cover which is applied weekly. The facility applied 374 cubic yards of soil
April 2018 and 462 cubic yards to date for May 2018.

Records Review
11. A copy of the current permit and operations plan are maintained in the landfill office.
12. 1st Semi-annual groundwater monitoring and 2nd Semi-annual groundwater monitoring for 2017 were reviewed.
Surface water monitoring is performed semi-annually with their groundwater monitoring.
13. Quarterly landfill gas monitoring is performed by Blue Flame LLC. 1st Quarter 2017, 2nd Quarter 2017, 3rd Quarter
2017, 4th Quarter 2017 and 1st Quarter 2018 were reviewed.
14. Random inspection reports were reviewed. Facility staff perform one (1) random inspection each day. Good
documentation of unacceptable waste found with documentation of final disposition.
15. Certified Operators are as followed:
Jeffrey Geiss, Certified Manager of Landfill Operations, expired February 11, 2019
Jeannette R. Ramos, Certified Landfill Operations Specialist, expires February 10, 2021
