The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. The Septage Land Application Site consists of Field 1 (13.8 acres), Field 2 (18.6 acres) and Field 3 (12.6 acres).
Looking at Field 2 from Field 3 2. The site is permitted for land application of domestic septage and grease trap pumpings.
3. The site had accessible all-weather roads to the septage land application field covered under the permit.
4. Appropriate signage was displayed at the entrance to the fields.
5. All fields had disposal area boundaries that were delineated with permanent markers which corresponded to
the site map. Current Set-back/buffer requirements appear to have been met.
6. All septage discharges appear to have been made within the disposal area boundary.
7. During the previous inspection, the fields had recently been seeded with winter rye. Provisions for winter rye is not in the approved nutrient management plan (NMP). A revised NMP will need to be done to include winter rye for the up-coming permit revision. Recent reseeding was observed. Weed control is in
progress and ongoing. At portion of Field 3 continues to have difficulties maintaining a fescue crop. The NMP requires areas that develop with less than 80% groundcover by fescue to be re-seeded with fescue.

8. No evidence of standing liquid or pooled waste was observed.
9. No objectionable odors were observed.
