1. Event Description
a. Date: discovered Wednesday, May 16, 2018
b. Material released
(Yes/No).
Description if yes: CCR material from the landfill
c. Summary:
• On Wednesday, May 16, the site experienced significant rainfall (2.63" total). 1.42"
of rain was received between 5:17 and 5:41 pm.
• Following the microburst rain event, it was discovered that the rainwater had pooled
around an chimney drain and overtopped the interim cover berm.
• The breach pushed protective cover soil, vegetative material and CCR material from
Cell 2 into the perimeter berm ditch but not outside of the approved process area. d. Field actions:
• The breached area was stabilized immediately until further repairs could be made.
• The operations contractor monitored stormwater pumping operations and landfill
stability overnight.
• On May 17, interim cover soil was placed in the breach area and compacted with a
dozer.

5. Duke Energy Follow-Up Actions:
• Clean up of the perimeter ditch is scheduled to be completed on May 17, 2018.
• A Prompt Incident Response Team (PIRT) is mobilizing to the site on May 17, 2018 to
provide a quick cause evaluation and provide guidance to prevent reoccurrence.
