The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Have received permit renewal. Need land application site logs from August 2017 until now.
2. The office has moved from the Rennert Road location here to the entrance of the site on Saddletree Road. Have
not yet received a key to the gate entrance as discussed on 12/14/2017. Mr. Locklear stated that he would send
one to me.
3. Approximately 2 acres were removed from Field 5 for the new compost area. Posted orange flags which
must be replaced with permanent makers. Re-GPS’d this field for a new site map to reflect the reduced
acreage on Field 5. The application rates/gallons must be adjusted on the permit holders land application
site logs for this change on this field.
4. Noticed what appeared to be a grease burn on Field 5, discussed this area with Eric Locklear who indicated it
would be repaired. Otherwise no sign of heavy or recent application. No odors noted.
5. Cropstand appears adequate at this time, noted some thin areas. Weed control needed. Continue efforts to
improve the bermuda per the advice of Georgia Love, regional agronomist, or your local extension agent. Only
effluent goes on the fields.
6. It appears that the approved setbacks are being met but were not measured at this time. No sign of land
application outside the permitted boundaries. Field markers appear accurate.
7. The required signs are posted and the access road is adequate.
8. Inspected the truck cleanout box with Mr. Locklear (container was mostly full & Mr. Locklear indicated that it
would be emptied soon). Discussed that measures should be taken to help prevent the waste spillage along the
top of the wall & between the wall and the box. The area should be cleaned up regularly, not just when the
cleanout box is emptied which can be months.
9. Soil samples will be taken by the permit holder this fall.
