The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. The third tank from the left that had a hole has been replaced with another tank of the same capacity. This new
tank may be used for septage detention. The tanks appear structurally sound.
2. It appears that septage is being transferred to/from the facility in a safe and sanitary manner. No leaks or spills
noted. No apparent vectors or odors.
3. It appears that required setbacks are being met with the currently permitted tanks.
4. Maintain the required 5’ cleared radius around the tanks. It is my understanding that the excess vegetation has
been sprayed in this area.
5. The required signs are posted and the access road is adequate. Work has been done at the site entrance at the
front gate (area enlarged) and also grading work for a truck traffic area in front of the shed & well house.
6. Two additional tanks have been placed at the far right side of the retaining wall. These two tanks shall
not be used for septage detention at this time. The normal setback distance of 100’ from the creek must
be verified and also how the high capacity requirements shall be met (double setbacks or containment). It
is my understanding that a survey shall be done and MacConnell & Associates shall be the engineering
firm handling the permit modification for the dewatering facility. A site visit is scheduled for May 24th.
