Unable to access Field 3 – post on left side of gate was broken/very unstable, could not safely open gate to access this field/was afraid of causing more damage to post. Contacted Randy Cheek by phone & told him of situation. He stated that he would take care of it. SEE BACK OF PAGE The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 2 ADDITIONAL COMMENTS 1. Could only inspect field 2. Field markers appear accurate. It appears that the approved setbacks are being met but were not measured at this time. No sign of land application outside the permitted boundaries. No sign of heavy or recent application. No odors noted. Weed control is needed this spring. Continue efforts to improve fescue. 2. A new site entrance (larger) has been installed. The access road is adequate. 3. The required signs are posted, however the sign on Field 3 is faded and needs repair/replacement. The sign for field 2 should be moved to the side of gate so that it is visible if the gate is open. 4. Soil samples will be taken by permit holder this fall.
