PURPOSE OF SITE VISIT:
Comprehensive Inspection
STATUS OF PAST NOTED VIOLATIONS: NA
OBSERVED VIOLATIONS: NA
ADDITIONAL COMMENTS
1. The purpose of this site visit was to conduct a comprehensive facility inspection.
2. Excellent sign at entrance to facility. 3. No unacceptable waste was observed at the time of this inspection.
4. Edge of waste markers were clearly visible. Very good.
The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

5. Side slopes and top of LCID were being regraded to achieve a 3:1 ratio at the time of this inspection.
6. No water was observed ponded over waste at the time of this inspection. Please, ensure that any areas that may hold water are remediated. 7. Area of erosion by sediment basin had been regarded since the last inspection. Ensure that slopes are graded
and sloped to prevent future erosion. 8. All large rocks/concrete shall be adequately covered or removed from slopes.
9. Vegetative ground cover sufficient to control erosion must be established within 30 (thirty) working days upon
completion of any phase of LCID development in accordance with Rule 15A NCAC 13B .0566(7) or as addressed in the approved Erosion and Sedimentation Control Plan and permit. 10. Ensure all trees and wood vegetation growing within the landfill foot print are properly removed. Trees less
than 6 inches in diameter may be bush-hogged. 11. Corrective measures are necessary as a result of the findings of the inspection and should be completed within
30 days’ receipt of this inspection report.
