STATUS OF PERMIT:
Active. Issued November 25, 2014. The Permit to Operate shall expire July 17, 2024. Pursuant to Rule 15A NCAC 13B .020l(g), no later than January 19, 2024, the permittee must submit a request to the Section for permit renewal and must update pertinent facility plans including, but not limited to, the facility operation plan and waste screening
plans. Pursuant to NCGS 130A-294(a2) and 15A NCAC 13B .0206(b), the Permit to Operate is subject to a limited
review by July 17, 2019. No later than January 19, 2019, the permittee must submit a request to the Section for a
limited permit review. A five-year limited review of a 10-yr permit includes review of the operations plan, closure plan, post-closure plan, financial assurance cost estimates, environmental monitoring plans, and any other applicable
plans for the facility.
PURPOSE OF SITE VISIT:

15A NCAC 13B .0105 COLLECTION AND TRANSPORTATION OF SOLID WASTE:
(3) The vehicle or container shall be equipped with seals, gaskets, or other devices pursuant to manufacturer specifications in order to prevent the escape of liquids. Such seals, gaskets, and other devices shall be maintained and replaced pursuant to manufacturer specifications.

Leachate was observed leaking from rear door seal. The seal should be repaired so this doesn’t happen again.
(7) The vehicle or container shall be serviced, repaired, and cleaned to maintain sanitary conditions, to preserve the integrity of the door seal, to prevent the accumulation of mechanical fluids, dirt, and filth on the vehicle's
exterior, and to prevent contamination of the environment by fluids. Due to the leaking door seal, leachate (animal blood) had leaked onto the ground.
15A NCAC 13B .0402 OPERATIONAL REQUIREMENTS:
(1) Operational plans shall be approved and followed as specified for the facility; Waste was left on the transfer station floor from Saturday until Monday.
(7) Appropriate method shall be provided to confine material subject to be blown by the wind within the area. At
the conclusion of each day of operation, all windblown material resulting from the operation shall be collected and returned to the area by the owner or operator;
The operator failed to pick up all the windblown material by the end the operating day.
ADDITIONAL COMMENTS; 1 – At the time of this inspection, leachate was observed leaking from the door onto the ground from Trailer
number 142. Leachate was not observed migrating offsite and appeared to be isolated to the area behind the trailer. The impacted soil was cleaned up right away and disposed of in the WTS. Trailers should be repaired to prevent leaking of leachate or removed from service.
2 – It may be necessary to keep some type of containers available to capture any leachate that comes from trailers with leaking gaskets or holes to prevent leachate from discharging onto the ground. I recommend that you inspect the trailers at your facility prior to loading for deficiencies
3 - Trailers are not being tarped when put on the yard holding area until the trailer is being hooked up to a tractor for transport to the MSWLF. This compounds the windblown trash issue. The Operation Plan received
on May 23rd. 2014 in section 2, states the trailers will be tarped when put in the holding area. 4 - Waste was left on the tipping floor from Saturday, April 21st. 2018 until found on Monday, April 23rd. 2018. Operations plan not being followed. Waste must be removed from the tipping floor and the tipping floor must be
cleaned at the end of working day. Site staff stated that waste was left on the floor as a result of no empty trailers being available. The contingency plan for this facility should address this type of situation and how it should be handled by staff. I recommend that you review your contingency plan and update if necessary to prevent this
type of issue in the future. 5 - Corrective action is needed from the results of this inspection. 6 - Digital Picture taken during this inspection with several attached to this report.
The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.
