OBSERVED VIOLATIONS:
1. While visiting the Durham septage treatment facility I was informed that there was a new vehicle there that
needed inspection. Vehicle tag#8032, VIN 1NKZX4EX4JJ211632, 4000 gallon capacity. The permit number
NCS-01374, phone numbers (1-877-263-8186 & 704-263-8186) and the firm name “Stanley Environmental
Solutions” were in compliance on both sides of the vehicle. However the town name displayed was “Stanley”
which is incorrect for this Durham firm permit. Please display “Durham” on both sides of this vehicle, three
inches or greater in height for all letters.

The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. For the new vehicle tag#ND8032 the outlet covers were on, no drips or leaks noted and the tank appeared
structurally sound & securely attached. No other vehicles were inspected at this time and truck logs were not
reviewed.
2. The recently submitted firm application lists 4 other vehicles for this firm permit.
Kenworth pump truck #SES57, 4000 gallons
Kenworth pump truck #SES64, 4000 gallons
Freightliner box truck #SES84, 600 gallons
Mack tractor #SES90, 6000 gallons
If this not correct please notify this office. Also please ensure that the town name displayed on all of the
pump vehicles is “Durham”.
