OBSERVED VIOLATIONS:
1. Permit Condition 7. “This facility consists of one 1,000 gallon pump truck stationed at an approved location in
Bay #1 and utilized for the storage of portable toilet waste”. During this visit it was noted that the approved
vehicle (tag#AH9184) was parked in a different location/adjacent shed (which was noted on a previous visit of
2/24/2017) and a private vehicle was parked in the approved location (Bay #1). Per this Septage Detention or
Treatment Permit the correct vehicle shall only be parked at the approved location (Bay#1). If the permit
holder wishes for the approved location to be changed please contact this office.

The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. The required “No Trespassing” sign was posted at Bay#1 and the access road was adequate.
2. It appears that septage is being transferred to/from the facility in a safe and sanitary manner. No spills or leaks
were noted where the correct vehicle was parked or in Bay#1. No apparent vectors or odors.
3. The truck tank on the approved vehicle (tag#AH9184) appears structurally sound.
4. The required 5’ cleared radius (free of excess vegetation or debris) is present.
