administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Inspected two new vehicles to be added to the firm permit. The $800 fee was submitted on 12/22/2017 for the
2018 firm permit (three trucks will be in operation).

Inspected vehicle tag#YA133604, VIN 1HTMKAAN73H574762, 2000 gallons
The tanks appeared structurally sound & securely attached to both trucks. Outlet covers were on and no
drips/leaks were noted. The lettering is correct on both sides of both vehicles. Both vehicles are in compliance.
2. Did not inspect the other pump truck listed on the firm application or review truck pumping logs at this time.
3. Discussed the process to obtain a permit for a septage detention tank. Enclosed is information for that permit.
