STATUS OF PERMIT:
Active; Permit to Operate Phases I and II and Permit to Construct Phase III issued 24 February 2016, expires 5
October 2020. A complete permit review request for the next 5-year phase of operations including updated Facility,
operations, and waste screening plans is due on or before 5 April 2020. This application must comply with the rules
and regulations in effect at the time of the application.
PURPOSE OF SITE VISIT:
Comprehensive Facility Compliance Inspection
STATUS OF PAST NOTED VIOLATIONS:
None applicable
OBSERVED VIOLATIONS:
No violations were observed during this Facility Compliance Inspection; please see the ADDITIONAL COMMENTS
section below.
ADDITIONAL COMMENTS:
1) This Facility is a Land Clearing and Inert Debris (LCID) Landfill.
2) This Facility is recorded with the Onslow County Register of Deeds at: Book #1794; Page #169.
3) This Facility is permitted to receive land clearing waste, yard trash, untreated and unpainted wood,
uncontaminated soil, and inert debris such as unpainted rock, brick, concrete, and concrete block.
4) This Facility is permitted to receive waste generated within Onslow County.

5) Signage is posted at the entrance to the Facility stating the Facility name, permit number, and Facility
emergency contact number.
6) The Facility is secured with steel posts at the entrance and a steel cable locked across the access road and has
controlled access for safety.
7) The Facility access road is of all-weather construction and is being maintained in overall good condition.
8) Soil has been stockpiled on-site for use in cover and grading operations.
9) Waste concrete and asphalt has been staged on the western side of the landfill access road for crushing and
later re-use. *Please note that the State rules regarding recovered materials state that for a material to qualify
as a recovered material, the majority of the material (75% or more) must be sold, used, or re-used within
twelve months.
10) The waste type being disposed at the working face is landclearing waste, however, a load of unpermitted
waste consisting of asphalt roofing shingles was observed disposed on the landfill away from the working
face. *Within thirty days of receipt of this Facility Compliance Inspection Report, please remove this
unpermitted waste from the landfill and transport it to a Facility permitted by the State to receive such wastes
(please see the aerial photograph attached on page three of this facility Compliance Inspection Report).
11) Waste has been pushed from the working face over the eastern sideslope. *Please compact and cover all
exposed waste on the eastern sideslope. The landfill should be constructed in a lift-type progression with all
outside slopes covered and maintained at a slope not greater than 3:1(please see the aerial photograph
attached on page three of this facility Compliance Inspection Report).
12) Overall, the outside slopes of the landfill have a well-established cover crop of native warm season grasses
and some coppice-type cover.
13) Some brush and other woody vegetation was observed growing in several areas of the landfill. To prevent the
growth of trees and other woody vegetation, a regular mowing schedule should be initiated. *Plans need to be
underway for the mowing of all landfill sideslopes and all other inactive areas of the landfill.
14) The area around the toe of the landfill needs to be cleared of brush and woody vegetation. *Within thirty days

of receipt of this Facility Compliance Inspection Report, please mow the area around the toe of the landfill
and maintain it clear of brush for maintenance and inspections.
15) The landfill perimeter needs to be staked to identify the permitted landfill footprint. *Within thirty days of
receipt of this Facility Compliance Inspection Report, please install permanent markers delineating the waste
disposal boundary and all required buffers.
16) No off-site erosion was observed during this Facility Compliance Inspection.
17) *Corrective actions are required as a result of this Facility Compliance Inspection.
