with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1- Inspected vehicle in regards to issuance of firm permit. Tag#KXD5003, VIN1FD0X5HT5HEC33115, waste
tank capacity is 650 gallons (Mr. Choffel corrected this on the application). The tank appears structurally sound
& securely attached. Outlet covers were on and no drips/leaks were noted.
2- Inspected the lettering displayed on this vehicle. The firm name, phone number & permit number were correctly
displayed on both sides of the vehicle. However, the town name (Maxton – which is where this Texas company
will operate from in NC) was less than three inches on both sides of the truck. Received photos on Friday
3/9/2018 which show that the town name has been corrected. Based on this information the vehicle is now in
compliance and the Raleigh central office was informed of the status.
3- No septage pump jobs shall be performed until the firm permit is issued by the Raleigh central office.
