STATUS OF PERMIT:
Active; Permit to Construct/Operate issued 6 September 2013 expires 6 September 2018. *A complete permit review
request for the next five-year phase of construction and/or operations is due on or before 6 March 2018. This
application must comply with the rules and regulations in effect at the time of the application. *For permit renewal

PURPOSE OF SITE VISIT:
Comprehensive Facility Compliance Inspection
STATUS OF PAST NOTED VIOLATIONS:
None applicable
OBSERVED VIOLATIONS:
No violations were observed during this Facility Compliance Inspection; please see the ADDITIONAL COMMENTS
section below.
ADDITIONAL COMMENTS:
1) This Facility is a Land Clearing and Inert Debris Landfill.
2) This Facility is permitted to receive land clearing waste, yard trash, untreated and unpainted wood,
uncontaminated soil, and inert debris such as unpainted rock, brick, concrete, and concrete block.
3) The Facility is secured by means of a locked gate at the Facility entrance and has controlled access for safety.
The gate was closed and locked upon my arrival and no attendant was on-site during this Facility Compliance
Inspection.
4) Proper signage is posted at the Facility entrance.
5) The Facility access road is of all-weather construction and is being maintained in overall good condition at

this time.
6) Soil has been stockpiled on the top lift of the landfill for use in cover and grading activities.
7) A Caterpillar D6K LGP Crawler Tractor is maintained on-site for compacting, cover, and grading operations.
8) The waste type was reviewed. The waste type being disposed is LCID waste and no unpermitted materials
were observed at the working face during this Facility Compliance Inspection.
9) The active working area is being managed in a compact and manageable size.
10) Portions of the top lift have recently been covered and graded. *Please continue to ensure that all waste is
covered with one foot of suitable soil monthly, or when the active working area reaches one acre in size,
whichever occurs first, or more often when necessary to prevent the site from becoming a nuisance or to
mitigate conditions associated with fire, windblown materials, vectors, or excessive water infiltration.
11) Outside slopes have been covered and graded at a slope not greater than 3:1. A cover crop of native warm
season grasses and mixed coppice-type cover has been established on the outside slopes. Small trees and other

woody-type vegetation were observed growing on the outside slopes of the landfill. *The outside slopes are in
need of mowing. Within thirty days of receipt of this Facility Compliance Inspection Report, please remove
all trees and other woody vegetation and mow the landfill cover crop.
12) The area around the toe of the landfill has been cleared to provide for free access around the site for Facility
maintenance and inspection. *Please continue to ensure this area is mowed and adequately maintained to
provide for clear access around the toe of the landfill.
13) No off-site erosion was observed during this Facility Compliance Inspection.
14) *Corrective actions are required as a result of this Facility Compliance Inspection Report.
