Expired, the Permit to Operate was issued on August 6, 2012 and is set to expire December 19, 2016. No later than July
19, 2016 Martin Co. must submit to the Section documents for a permit amendment. Permit renewal in process.

The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS:

01 - Expired, the Permit to Operate was issued on August 6, 2012 and is set to expire December 19, 2016. No later than
July 19, 2016 Martin Co. must submit to the Section documents for a permit amendment. Documents sent. On April 10,
2017 comment review was sent to Smith/Gardner (Pieter Scheer) from Donna Wilson (permitting engineer). Permit
renewal in process, per Donna Wilson (919-707-8255) on February 26, 2018. Need to have consulting company check
status if not soon received.
02 – Facility is secured by a Chain Link Fence and Locking gate.
03 – Access roads are made of all-weather materials.
04 – Signage meets permit requirements.
05 – Facility has 4 Certified Landfill Operations Specialist: Johnny Ray Roscoe: expires 12/06/2018, Ronald G.
Brownfield: expires 10/19/2019, Kimley L. Brown: expires 09/27/2019 and Justin K. Harrison: expire 09/27/2019.
06 – Incoming load weight tickets have required information.
07 – Waste screening is being conducted and records have required information.
08 – Cover logs are being done and are up to date.
09 – Tires received are being laced into a closed top trailer.
10 – Yard Waste area is being managed well and has only permitted materials onsite.
11 – All Inert Debris are in compliance.
12 – Scrap Metal and White goods area is being well.
13 – All White Goods containing CFC’s are being separated from other metals and are standing upright until CFC is
removed.
14 – Facility has CFC removal records.
15 – 5901-MSWLF-1985 (Closed) was in need of material added to low areas on the top to prevent the possibility of
leachate issues from the head pressure on these areas.
16 – Groundwater monitoring well #2 has an issue getting the required sample deeper that 5 to 6 feet. Have consulting
Company discuss plans with DWM – Permitting Hydrogeologist (Christine Ritter @ 919-707-8254).
17 – Corrective action needs to be taken upon this facility inspection. Listed on this inspection report.
