STATUS OF PERMIT: Active Issued: 7/02/2014 Expired: 12/16/2018 PURPOSE OF SITE VISIT: Routine STATUS OF PAST NOTED VIOLATIONS: N/A OBSERVED VIOLATIONS: N/A The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 2 ADDITIONAL COMMENTS 1. All photographs were taken by Troy Harrison on June 13, 2017. 2. The Septage Land Application Site consists of Field 1 (13.8 acres), Field 2 (18.6 acres) and Field 3 (12.6 acres). 3. The site is permitted for land application of domestic septage and grease trap pumpings. 4. The site had accessible all-weather roads to the septage land application field covered under the permit. 5. Appropriate signage was displayed at the entrance to the fields. 6. All fields had disposal area boundaries that were delineated with permanent markers which corresponded to the site map. Current Set-back/buffer requirements appear to have been met. 7. All septage discharges appear to have been made within the disposal area boundary. Recent land applications were made in Field 1. 8. No evidence of standing liquid or pooled waste was observed. 9. No objectionable odors were observed.
