administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Inspected vehicle in regards to issuing first firm permit. Tag#MW2972, VIN 1FVXJJCB5YHG77869, 3000
gallons. Tank appears structurally sound & securely attached. Outlet covers were on and no drips/leaks were
noted. The lettering is correct on both sides of this vehicle. The firm name on the passenger side is slightly
damaged but still legible at this time.
2. Mr. Ammons gave me the original notarized WWTP authorizations for PWC & Lumberton. I sent both to the
Raleigh central office by scan & by courier mail. The Raleigh central office is waiting to receive a $50 check
from EHC Environmental towards the required fee (it was mailed earlier this week). Discussed with Mr.
Ammons that no septage pump jobs shall be performed until the permit is issued by the Raleigh central office.
