The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. The tank appears structurally sound. Mr. Driggers discussed the possibility of cutting a reasonable opening in
the top to allow for better access (currently the tank only has a front bottom outlet).
2. The area around the tank has been cleared completely/raked for a radius of 5 feet. Discussed that groundcover
(gravel, low vegetation) was permitted around the tank so that the tank sides & surrounding area were visible.
3. It appears that septage is being transferred to/from the tank in a safe and sanitary manner. No leaks or spills
noted. No vectors or odors.
4. The approved setbacks are being met. A shed has been built nearby but no well.
5. The required sign is posted. The access road is adequate.
6. Reviewed the SDTF application submitted for permit renewal, made minor revisions. Mrs. Brenda Driggers (a
landowner along with Charles Driggers) also signed the application form so a landowner authorization form is
not required. The new SDTF permit will be issued under both Brenda & Charles Driggers.
