enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Inspected pump truck in regards to issuance of first firm permit. Tag#YA143174, VIN 3FEXF8015XMA14785,
2000 gallons. Tank appears structurally sound & securely attached. Outlet covers were on and no drips/leaks
were noted. The lettering is correct on both sides of this vehicle.
2. I received an original notarized WWTP authorization form which I sent to the Raleigh central office by scan &
courier mail. No septage pump jobs shall be performed until the firm permit has been issued by the Raleigh
central office. When operation commences please begin maintaining the required information for the truck
pumping log.
