Issued on October 30, 2013 and expires on December 12, 2018. Recorded in Currituck County, NC, Register of Deeds - Book 875 and Page 807. PURPOSE OF SITE VISIT: Comprehensive Facility Compliance Inspection STATUS OF PAST NOTED VIOLATIONS: None OBSERVED VIOLATIONS: None FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 3 ADDITIONAL COMMENTS; 01 - Facility has adequate security. Locking gates and fencing. 02 - Facility has adequate signage with all required information. 03 - Interior roadway is constructed of all-weather materials. Paved and rock/gravel roads. 04 - This facility is owned by Currituck Co. but is operated by a contract company. The Bowen Company, Inc. is the contractor. 05 - Facility has certified operators that are employed by The Bowen Company, Inc. Two onsite during this inspection. William Etheridge – expires May 20, 2021 and James Bolen – expires October 09, 2020. 06 - Records were available for review. 07 - Random inspections are being conducted and recorded. 08 - The tipping floor is kept in a sanitary condition. 09 - Waste is deposited on the tipping floor and then pushed into trailers. 10 - Stored trailer area looked good today. 11 - Windblown litter was at a minimal during this inspection. Some noted at the ends of the loading area. 12 - The trailer loading area needs some maintenance on the entrance and exit ends to prevent any wash down water from draining onto the ground. Have staff start at ends and push all water to drains so all water goes through the leachate system. Permit condition Part VI on page 8 number 17. 13 - Solid waste received are weighed in at the scale house. Records were review and found to have all required information. 14 - Waste is transported to the East Carolina Regional Landfill, (Permit Number 08-03-MSWLF-1993) in Aulander, Bertie County, North Carolina. 15 - Scrap tires are laced into trailers and removed. 16 - Yard Waste area appears to be managed well. Be sure to have plastic bags removed or any other non-approved waste that may be slipped in under the yard waste. 17 - White Goods and scrap metal are being pushed up into a large pile. This is being conducted before the Chlorofluorocarbon (CFC’s) refrigerants are being removed. Need to source separate white goods containing the CFC’s and keep the upright until the CFC has been removed to prevent any environmental issues. 18 - Tipping Floor fire control measures consist of 3 fire extinguishers and 2 water hoses. There is also 1 extinguisher located at the Yard Waste / White Goods area. 19 - Corrective action is needed on Trailer loading and White Goods areas due to this facility inspection. 20 - Digital pictures taken and attached to this report. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.
