The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. The Septage Land Application Site consists of Field 1 (10.0 acres), Field 2 (3.8 acres) and Field 3 (5.1 acres) for a
total of 18.9 usable acres for the land application of domestic septage (including portable toilet waste) and grease
septage.
2. The site had accessible all-weather roads to the septage land application field covered under the permit.
3. Appropriate signage was displayed at the site.
4. All fields had disposal area boundaries that were delineated with permanent markers which corresponded with the
aerial photograph submitted with the permit application.
5. All septage discharges appear to have been made within the disposal area boundary.
6. No evidence of standing liquid or pooled waste was observed.
7. Field 1 had a crop of fescue; however, the weeds are beginning to take over and may require re-seeding according
to the Nutrient Management Plan (NMP). In Field 2 the summer millet crop had past the end of its life cycle but
still being used. The NMP needs to be followed for this field as well. There were several areas in Field 2 where
heavy application may have resulted in damage to the crop. Field 3 may also require weed control and re-seeding
to maintain the required 80% crop ground cover.

8. No objectionable odors were observed.
9. Land application records were not available. As a reminder, land application records must be made available
upon request.
