1. Event Description
a. Date: discovered Monday Feb. 5, 2018
b. Material released
(Yes/No).
Description if yes: landfill leachate & primary basin dewatering water from the
onsite tank
c. Summary:
• On Friday, Feb. 2, operation of the system to pump from the leachate tank to the City of Eden was started.
• On Saturday, Feb. 3 it was noted that there was an excess amount of water in vault 11 (last manhole on force main from tank to city). The water was pumped out of the
vault but no leak was noted.
• On Monday, Feb. 5 it was noted that the air release valve (ARV) in vault 11 had a
small drip leak. No water was released to the environment as all of the water was contained within the vault. d. Field actions:
• A replacement ARV was ordered on Monday, Feb. 5.
• The ARV vaults were monitored when the system was in operation on Saturday,
Monday and Tuesday.
• Water was pumped from the vault and placed back into the leachate collection
system.
• The ARV in Vault 11 was removed on Feb. 6 and a plug has been put in its place.

• An evaluation of all of the ARVs onsite is being conducted to determine if the ARVs are
