HomeMy WebLinkAbout8505_Duke_Belews_IncidentReport_DIN25781_201603181 Frost, Larry From:Witt, Kimberlee <Kimberlee.Witt@duke-energy.com> Sent:Thursday, March 17, 2016 11:13 PM To:Frost, Larry Cc:Sullivan, Ed M; Andrews, Evan; Harrison, Will Michael; Martin, Melonie Y; Mussler, Ed; Mckee, Shawn Subject:Duke Energy Belews Creek Steam Station FGD Landfill (Permit No. 8505-INDUS) - Liner Incident Attachments:2012-02-17 Belews FGD Landfill Mining.PDF; 2016-03-17 BCSS Incident Photo 1.jpg; 2016-03-17 BCSS Incident Photo 2.jpg; 2016-03-17 BCSS Incident Photo 3.jpg Larry‐    As a follow up to our conversation earlier today:    Belews Creek Steam Station (BCSS) FGD Landfill (Permit No. 8505‐INDUS)    Event:   At approximately 10:15AM, Duke’s landfill contractor was performing gypsum mining operations in the FGD  Landfill at BCSS when the liner system was damaged by an excavator   The liner system damage is approximately 5‐ft long by 4‐ft wide and includes the overlying geocomposite  drainage layer, the geomembrane, and the underlying geocomposite clay liner   Photos are attached to this email for your reference    Actions Taken   Gypsum mining was immediately suspended   The damaged liner area was cleared of gypsum   The damaged liner area was inspected by Duke internal engineers   Provisions were made to minimize run‐on to the area   A tarp was placed over the damaged liner area to prevent any surrounding material from encroaching    Actions Intended   A representative from Joyce Engineering will be onsite tomorrow morning (3/18) to inspect liner damage and  oversee repair (complete CQA)   A liner crew will be onsite tomorrow morning (3/18)   The liner repair area will remain open for NCDEQ inspection on 3/23   A CQA report of the liner repair will be submitted to NCDEQ Solid Waste   The FGD Landfill Operations Plan will be revised to include an operational standard for mining of gypsum   Mining at the FGD landfill will remain suspended until a revised Operations Plan has been approved by NCDEQ  Solid Waste   Duke Energy will work with NCDEQ to ensure that sufficient “mining” language is included in the revised  Operations Plan    General information:  Mining at the FGD landfill was first implemented in February of 2012.  The attached copy of an email from Ed Sullivan to  Larry Frost dated 2/17/2012 contains confirmation of planned mining activity discussions.  Below you will find language  recently added to various Duke Energy Landfill Permits.  Since the BCSS FGD Landfill contains potentially all mineable  gypsum, this typical language will be altered as needed to ensure safe operations for mining specific to the FGD Landfill. 2   Please let Melonie or me know if you have any additional questions‐  Kim    Kimberlee Witt, PE DUKE ENERGY Sr. Engineer - Environmental Services 3171 Pine Hall Rd. In order to maintain an uninterrupted supply of gypsum to the wallboard manufacturer, it may be necessary in the coming weeks and months to mine gypsum from the landfill due to low projected gypsum production rates related to low operating hours for coal-fired generation during this period of mild weather. Per our conversations today, it is my understanding that there are no permitting issues associated with this activity. In addition, it is our understanding that the gypsum removed from the landfill will be weighed and tracked in a similar fashion to the weighing of gypsum destined for disposal. The annual landfill report will include the gypsum disposed as well as the gypsum removed. Also, it is NCDENR Winston-Salem Regional Office’s desire to be notified when the mining begins so that a site visit can be scheduled. I appreciate the assistance that you provided today.
