143-215.3D(a)(4). You are receiving this notice because the annual fee of $100.00 for coverage under NPDES General
Stormwater Permit NCGO10000 for Construction Activities has not been paid. The annual fee was billed on 12/1/2020 and was
due on 1/1/2021.

of Violation and potential civil penalty assessment of up to $25,000 per day for failing to maintain permit coverage. Should you
