REGAN Secretary LINDA CULPEPPER Director Mr. Dennis Poythress Dixie Grocery, Inc. 4365 NC Hwy 39 S Henderson NC 27537-7524 Dear Mr. Poythress: Nc�GPTH CAROL+r.A Fn vlronmenral Quali ry September 23, 2019 Subject: NPDES Compliance Evaluation Inspection Groundwater Remediation System (Petroleum) General Permit NCG510000 Certificate of Coverage NCG510070 Dixie Grocery (4730 NC Hwy 39 S, Gillburg) Vance County On September I0, 2019 Jason Robinson of the NC Division of Water Resource's Raleigh Regional Office (RRO) conducted a compliance evaluation inspection at the former site of Dixie Grocery at 4730 NC Hwy 39, S, Gillburg, specifically for compliance with General permit NCG510000 and Certificate of Coverage NCG510070 that covers the groundwater remediation discharge system. We thank Mr. James Dobson of EMS Environmental for his correspondence and Mr. Justin Abreu of EMS for meeting the inspector onsite. Based on the inspection, correspondence with Mr. Dobson and a file review, the following was determined: I. This Certification of Coverage (CoC) is for the operation of a groundwater remediation system and discharge of treated petroleum -contaminated groundwater or similar wastewater stream. The CoC was originally issued on June 30, 1993, was reissued on November 21, 2016 and will remain effective for the duration of the General Permit. 2. The pump and treat system that is covered under the general NPDES permit has not been operational since August 9, 2018. Mr. Dobson explained that the groundwater system was deactivated at this time as directed by the NCDEQ UST Section. As of now, a rescission of the CoC has not been requested, pending a decision by the UST Section. 3. Mr. Dodson provided the Discharge Monitoring Reports (DMRs) and lab bench sheets for the last quarterly sampling that was done (June 2018). No discrepancies were found. 4. The operator uncovered the end of the pipe in the ditch. It was dry with no evidence of discharge.
