On August 7, 2015, the Division of Energy, Mineral and Land Resources received your request to rescind
your coverage under Certificate of Coverage Number NCG080040. In accordance with your request,
Certificate of Coverage Number NCG080040 is rescinded effective immediately.
Operating a treatment facility, discharging wastewater or discharging specific types of stormwater to waters
of the State without valid coverage under an NPDES permit is -against federal and state laws and could result
in fines. If something changes and your facility would again require stormwater or wastewater discharge
permit coverage, you should notify this office immediately. We will be happy to assist you in assuring the
proper permit coverage.
If the facility is in the process of being sold, you will be performing a public service if you would inform the
new or prospective owners of their potential need for NPDES permit coverage.

voice mail to you this morning, I know that Waste Management (Shawn Carroll) submitted a "permit name/ownership
change request" form for our Fayetteville Hauling Company in a letter addressed to you, dated March 27, 2015. Our

intent in that correspondence was to simply change the responsible contact within our organization. I believe that the
form we submitted will accomplish that, but please let me know if you would prefer that we submit the "permit owner

General Permit are outlined in the attached Technical Bulletin.
Your coverage under the General Permit is transferable only through the specific action of DWQ.
This permit does not affect the legal requirements to obtain other permits which may be required by
DENR, nor does it relieve the permittee from responsibility for compliance with any other applicable
federal, state, or local law, rule, standard, ordinance, order, judgment, or decree.

Your facility is currently covered for stormwater discharge under General Permit NCG080000. This permit expires on 10J31/2012.
The Division staff is currently in the process of renewing this permit. When a draft is available for notice and public comment, it will
be posted on our website at http://portal.ncdenr.org/-,veb/wq/ws/su/public-notices. Please continue to check this website to review
and comment on proposed changes to the permit. Once the permit Is reissued, your facility would be eligible for continued coverage
under the reissued permit.
To assure consideration for continued coverage under, the general permit, you must apply to the Division of water Quality (DWQ) for
renewal of your permit. Enclosed you will And a Permit Coverage Renewal Application Form. The application must be completed
and returned by May 4, 2012. Letters confirming our receipt of the completed application will not be sent.
Failure to request renewal within the time period specified may result In a civil assessment of at least 5250.00. Larger penalties may
be assessed depending on the dcliriyuency of the request. Discharge of stormwater from your facility without coverage under a valid
stormwater NPDES permit would constitute a violation of NCGS 143-215.1 and could result In assessments of civil penalties of up to
525,000 per day, our records indicate that your facility has outstanding annual fees in the amount of $100. We cannot
renew permit coverage until you have corrected this situation. We have enclosed copies of the unpaid Invoice(s) that must be
returned with payment. Failure to pay annual fees is a vitiation of your permit and may result in revocation of your permit coverage.
"No exposure exclusion" is now available to industrial facllitles in any of the categories of "storm water discharges associated with
Industrial activity," (except construction activities). If you Feel your facility can certify a condition of "no exposure", i.e. the Facility
industrial materials and operations are not exposed to stormwater, you can apply for the no exposure exclusion. For additional

If the subject stormwater discharge to waters of the state has been terminated, please complete the enclosed Rescission Request

Payment of the required annual administering and compliance monitoring fee of 580,00 for this year has not
been received for the subject permit. This fee is required by Title 15 North Carolina Administrative Code 2H.0105, under
the authority of North Carolina General Statutes 143-215.3(a)(1), (1 a) and (11 b). Because this fee was not fully paid within
30 days after being billed, this letter initiates action to revoke the subject permit, pursuant to 15 ncac 2H.0105(b) (2) (k)
(4), and G.S. 143-215.1 (b) (3).
Effective 60 days from receipt of this notice, subject permit is hereby revoked unless the required Annual

Jim and I spoke about 3:45 today, and he asked for some information on the
check from Houston. Please share this note with Jim. See the second page
which shows:
a) a photo -copy of the certified mail green card showing that Nikki
Cannoy(sp?) signed for the NOV letter addressed to Gordon Leonard in
Mount Airy, and
b) a photo -copy of the check from Houston for $80.
Right now, we are thinking that the first notice went out 10/25/03 to the Gordon
Leonard address. If it was returned to us by the post office as undeliverable, or
wrong address, then Fran (NCDENR accounting) would go back in her files for
another address. We're thinking that probably she did that, found the Houston
address, and sent a second, reminder notice to Houston sometime after
11/22/03, the original due date. Our theory now is that they got it, and paid it,
only as you see, very late, in March. But, before they paid it in March, we sent
out the February NOV for non-payment of the invoice which was originally due
in November.
Jim agreed to call me next Tuesday to see if we can straighten this out.
