requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement between the
state of North Carolina and the U.S. Environmental Protection Agency, dated October 15, 2007 (or as
subsequently amended.) The following information is included with your permit package:

• Annual Discharge Monitoring Report Forms (if applicable)

The General Permit authorizes discharges of stormwater only, and it specifies your obligations with respect
to stormwater discharge controls, management, monitoring and record keeping. Please review the new permit
to familiarize yourself with all changes in the reissued permit. Significant changes to the General Permit
are outlined in the Technical Bulletin.
Your coverage under the General Permit is transferable only through the specific action of DEMLR. This
permit does not affect the legal requirements to obtain other permits which may be required by the N.C.
Department of Environment and Natural Resources, nor does it relieve the permittee from responsibility for
compliance with any other applicable federal, state, or local law, rule, standard, ordinance, order, judgment
or decree.

The permit does appear to be active, but the COC you attached is old. The general permit expired and was renewed in
2013. Our records show that this COC was NOT renewed electronically because at that time, this COC had been
expired. It was also NOT part of the batch that was automatically renewed in 2013, which may mean fees were not
current at the time of renewal. I do not find a record of a paper COC or package for NCG130048 irf our files (paper or
electronic).
Our database indeed shows an active permit with fees up to date since 2008, so this facility DOES have current
coverage. However, there should be a new COC that was effective on June 1, 2013—unless there is a record of renewal
on a later date I'm unaware of. We will look into this and follow up with you. We will need to generate a new COC for
this facility if we cannot locate one made at the time of renewal.
In the meantime, please be sure the facility has all the appropriate Permit documents from the 2031 renewal. You can

2013, so I'm not sure why there is no paper package record. I believe our on-line system will only generate COCs

This is a general message but specifically in reference to our client Central Carolina Holdings, Permit Number
NCG 130048.
Am I correct in interpreting that the COC is renewed, deemed "Active" and current, when the General Permit was
renewed (6/1113), as represented in the 'Active & Expired NPDES Permits' worksheet on the DENR website.
This has been our position with this permit; by way of the COC being issued August 7, 2008 and reference to the
GP for expiration.
I request this clarification on behalf of the owner as part of a Corporate Compliance project; I would greatly
appreciate your assistance.

On May 8, 2013, a site inspection was conducted for the Central Carolina Holdings, LLC facility located at 1616
McKoy Town Road, Harnett County, North Carolina. A copy of the Compliance Inspection Report is enclosed for
your review. Permit coverage authorizes the discharge of stormwater from the facility to receiving waters
designated as UT to Cypress Creek and a UT to Buffalo Creek, a Class WS stream in the Cape Pear River Basin.
The site visit and file review revealed that the subject facility is covered by NPDES Stormwater General Permit-
NCG130000, Certificate of Coverage-NCG130048.
As a result of the inspection, the facility was found to be in compliance with the conditions of the NCG130000
permit. Please refer to the enclosed Compliance inspection Report for additional comments and observations
made during the inspection.
Please be advised that violations of the NPDES Stormwater General Permit are subject to a civil penalty

The facility has the required SPPP, but needs to be signed and dated. Also, there is a sheet in the front of the plan that
documents yearly updates. Make sure to fill out this part of the plan yearly, and make any corrections that need to be
made.
Sampling was conducted according to the permit requirments.
Analytical monitoring is not required because vehicle maintenance is conducted in a maintenance building, and all fluids
are kept under this buildings roof.
All outfalls were visited except 004A, 0048, 004C, and 003. There are a total of 12 outfalls on -site.

In response to the Notice of Violation dated August 3, 2009 and associated inspection report
from July 28, 2009, Central Carolina Tire (CCT) Disposal offers the following information and
corrective actions.
1. OCTupdated the Storm Water Pollution Prevention Plan (SWPPP) for the facility in
August of 2008, and has performed an annual review in August 2009. Based on the
annual review, only minor updates were required to include a recent inspection report
and the analytical sampling waver issued by NCDENR-DWQ in August 2008.
2. CCT has contracted with the SWPPP preparer, Garrett & Moore, Inc., to assist in fully
implementing all aspects of the SWPPP. This includes all required qualitative sampling
of stormwater outfalls.
3. Garrett & Moore, Inc. will conduct qualitative sampling during the next Representative
Storm Event, and will continue to conduct qualitative sampling at the required semi-
annual frequency. Results from the qualitative sampling events will be recorded in the
SWPPP.
We trust that this response adequately addresses the issues raised in the Notice of Violation.

Our office is in receipt of your letter dated August 25, 2008 concerning the analytical monitoring
requirement for facilities with onsite vehicle maintenance activities covered under the NCG130000
general stormwater permit. According to the information provided in your letter, all vehicle maintenance
activities are conducted in the maintenance buildings onsite within the drainage area for outfall 006.
Also, the maintenance buildings are without floor drains and fluids are kept under cover and within
secondary containment. Your letter goes on to state that stormwater runoff does not come in contact
with the vehicle maintenance areas. Based on this information as well as a prior site visit conducted by

Central Carolina Tire (CCT) Disposal operates a scrap tire processing facility and monofill in
Harnett County, near the town of Cameron, North Carolina. CCT recently submitted a Notice of
Intent for coverage under North Carolina NPDES General Permit Number NCG130000. The
purpose of this correspondence is to request NCDENR's recognition of Representative Outfall
status as defined in Part Ill, Section D.5 of the General Permit for Outfall 004 and Outfall 005 at
the facility.
Outfall 004
As shown on the attached Figure 1, the drainage area for Outfall 004 has three individual
outfalls, designed Outfall 004A, 004B, and 004C. Industrial activity within the drainage area is
limited to scrap tire disposal and covering in the tire monofill. The individual outfalls all consist
of stormwater detention ponds with engineered outlets. Stormwater is diverted from the
drainage area to the stormwater ponds by way of engineered, grass -lined stormwater channels.
Because the upstream activities for each of Outfall 004A, 004B, and 004C are identical, and
each outfall consists of a similar outlet structure, stormwater discharge from any of the three
outfalls is expected to be substantially identical, Based on site accessibility and potential for
representative discharge during a storm event, we request that Outfall 004C be recognized as
the Representative Outfall for this drainage area. Outfall 004C will be qualitatively monitored
according to the frequency required by the General Permit,

Outfall 005A, 005B, 005C, and 005D. Industrial activity within the drainage area is limited to soil
stockpiling and borrow in support of onsite operations. Outfall 005A is a vegetated swale fed by
associated roadway shoulder ditches. Outfall 005B, 005C, and 005D are engineered sediment
traps with rock filter outlet structures. Stormwater is diverted from the drainage area to the
sediment traps by way of vegetated channels and berms.
Because the upstream activities for each of Outfall 005A, 005B, 005C and 005D are identical,
stormwater discharge from any of the four outfalls is expected to be substantially identical.
Based on site accessibility and potential for representative discharge during a storm event, we
request that Outfall 005C be recognized as the Representative Outfall for this drainage area.
Outfall 005C will be qualitatively monitored according to the frequency required by the General

Central Carolina Tire (CCT) Disposal operates a scrap tire processing facility and monofill in
Harnett County, near the town of Cameron, North Carolina. CCT recently submitted a Notice of
Intent for coverage under North Carolina NPDES General Permit Number NCG130000. The
purpose of this correspondence is to confirm that the CCT facility is not subject to the on -site
vehicle maintenance monitoring requirements described in the General Permit.
Referring to the attached Figure 1, CCT performs all vehicle maintenance in the maintenance
buildings on site within the drainage area for Outfall 006. Vehicle maintenance activities are
performed in permanent buildings without floor drains, and maintenance fluids are kept under
cover and with secondary containment as appropriate. Based on the arrangement of the
maintenance facilities, stormwater runoff does not come in contact with vehicle maintenance
areas at the site, and therefore does not leave the site through Outfall 006, CCT Teeks- yU,u
concurrence that Outfall 006 dne ! not require analytical- sampling based on the information
presented above.

Conducted a site visit today to the subject facility. Met with their environmental
person and engineer. They are very aware and up to speed with the NCG13 permit
conditions and we have no objections to issuing them coverage under the permit. As
an FYI, the site actually has 11 total outfalls, but the engineer has identified six
(as noted on the NOI) that would be representative. I advised him that he needed to
submit a letter to me requesting such representative outfall status so that I could
