Enclosed please find a copy of the Compliance Evaluation Inspection form for the stormwater inspection
conducted by Mack Wiggins of the Raleigh Regional Office on December 6, 2012. The assistance offered by Mr.
Donald Crowder and Mr. William Hux was appreciated. The facility was found to be in Compliance with
stormwater permit NCG 110000 at the time of this inspection.
1. The following stormwater observations were noted:
1. The Stormwater General Permit Certificate of Coverage (NCG 1 10086) was issued to this facility on
June 1, 2008 to the Town of Weldon's W WTP. The General Permit expires on May 31, 2013.
2. Weldon's Wastewater Treatment Plant has one stormwater outfall. This outfall was observed during
the inspection. The outfall was assessable for sampling at the time of the inspection.
3. The facility does not perform vehicle maintenance activities on -site which uses more than 55 gallons
of new motor oil when averaged over a calendar year. Therefore, only qualitative monitoring is
required. Qualitative monitoring was conducted as required in NCG I 10000.
4. The Stormwater Pollution Prevention Plan (SPPP) has been implemented. The plan was complete and
comprehensive.
5. All chemicals have secondary containment.

On October 28, 2009, Myrl Nisely of the Raleigh Regional Office conducted a compliance inspection of
the stormwater program for the Weldon WWTP facility with assistance from ORC Donald Crowder and
Backup ORC Bill Hux. The program was in compliance. Attached is a checklist containing inspection
details.
The Stormwater Pollution Prevention Plan manual had been maintained and showed evidence of an
active and fully implemented program. Employee training is yet to do by year's end. Good
housekeeping and attention to careful sludge handling during Synagro hauling help protect the
stormwater discharged from the site.

the stormwater program for the Weldon WWTP facility with the help of ORC Donald Crowder. The
facility was in compliance. Attached is a checklist containing inspection details.
The Stormwater Pollution Prevention Plan manual had been maintained and showed evidence of an
active and fully implemented program. Employee training and semi-annual qualitative observations had
been carried out, for example. The most significant potential threat to stormwater occurs during
episodes of sludge transport for land application.

On March 11, 2008, Mr. Myrl Nisely and Mr. Cheng Zhang of the Raleigh Regional Office conducted a
compliance evaluation inspection of the Stormwater program at the Weldon Wastewater Treatment Plant with
the help of ORC Donald Crowder. The facility was in compliance.
Description: Stormwatcr runoff is collected via several drop inlets, combined with flow from off -site, and is
discharged into a short swale to the Roanoke River, a class C stream in the Roanoke River basin.
Observations: Attached is a checklist for the inspection. All components of the Stormwater Pollution
Prevention Plan manual were present and up to date. Semi-annual qualitative observations have been carried
out, as well as annual employee training.

This Stormwater Pollution Prevention Plan is prepared in accordance with and follows
the Format of Part II Monitoring, Controls, and Limitations for Permitted Discharges,
Section A: Stormwater Pollution Prevention Plan found on pages Part II 1 through 5 of
the Town of Weldon's General Stormwater Permit No. NCGI10000 issued by the NC
Division of Water Quality on September 23, 2005.
1. SITE PLAN
(a.) A general location map of the wastewater treatment plant existing facilities is
provided in Figure I which shows the location of the wastewater treatment
plant in relationship to transportation routes and the receiving water to which
the stormwater outfall discharges, the Roanoke River. The latitude and
longitude of the discharge point is 36' 25' 24" and 77' 34' 39" respectively.
(b.) The Town of Weldon stores chemicals at the wastewater treatment plant that
are necessary for the operation and maintenance of facility.

Small amounts of other chemicals are stored on site such as reagents in the
laboratory, cleaning supplies, etc. that will not be included in this report.
The turbine oil is purchased by the operations personal, brought to the plant
site by them and stored in the Operations Building. The chlorine and sulfur
dioxide 150 pound containers are brought to the plant site by a commercial
truck and offloaded to the Chemical Feed Facility. The same is true for
sodium hydroxide (caustic soda). Only quantities necessary for immediate
use are stored on site. There is no long term storage of these or other
chemicals on site.
There are no chemicals stored outside exposed and unprotected. There are no
outside process areas involving chemicals. Chemical use is contained within
dedicated structures. No dust or particulates are generated at this facility.

Waste sludge is stored in an in -ground concrete sludge holding basin. A
licensed sludge disposal company pumps the sludge from the holding basin
into trucks for land application off site.
(c.) Figure 2 shows the existing stormwater system to scale as well as the existing
wastewater treatment plant facilities. The storm water system consists of 11
catch basins; 15 and 30 inch diameter stormwater pipe; one sediment trap,
several rock check dams and one discharge to the Roanoke River. The
latitude and longitude of the discharge point is 36' 25' 24" and 77' 34' 39"
respectively.
Stormwater from a private farm upstream of the wastewater treatment plant is
captured in catch basin # 1 and conveyed around the wastewater treatment
plant. All other stormwater within the wastewater treatment plant is captured
in the catch basins and conveyed offsite through the sediment trap to the
discharge point.
(d.) There have been no spills or leaks at the wastewater treatment plant during the
3 previous years.
(e.) Certification that the stormwater outfall has been evaluated for the presence of
non-stormwater discharges is found in Exhibit A. Non-stormwater discharges
have not been found at the outfall.

2. STORMWATER MANAGEMENT PLAN
(a.) Feasibility Study
An onsite evaluation was conducted of the chemicals utilized at the
wastewater treatment plan, the method of operations and storage practices
employed on site. The Town of Weldon's operation staff is properly
managing ,the chemicals stored on site. There is no direct potential
contamination of stormwater by the chemicals utilized on site. No changes to
the existing operations and storage practices are recommended.
(b.) Secondary Containment Schedule
There is no requirement for a secondary containment schedule for the bulk
storage of liquid materials at the Weldon wastewater treatment plant.
(c.) Best Management Practices (BMP) Summary
No non-stormwater discharges were identified at the existing sediment trap or
discharge point. All chemicals used at the wastewater treatment plant are
properly managed and stored by the operations staff. No spills or leaks have
occurred within the last three (3) years at the wastewater treatment plant. No
structural BMPs are necessary at the wastewater treatment plant at this time.
The following non-structural BMPs are recommended as good practice to
follow by the operations staff on a continuous basis.

Table 2: Non -Structural BMPs
1. Eliminate exposure of materials and equipment wherever possible by
moving materials and equipment to indoor locations.
2. Practice good housekeeping on -site. Keep materials handled and
stored at the facility in an orderly fashion.
3. Examine process and exchange hazardous materials for non -hazardous
ones wherever possible.
4. Establish routine leak and maintenance checks to minimize the chance
of spills before they occur. When spills do occur, clean them up
immediately.
5. Establish bulk storage tank protocols that minimize the risk of spills
during loading and unloading procedures.
6. Store used pallets and process waste dumpsters inside or under a roof
where water cannot flow on or around them.
3. SPILL PREVENTION AND RESPONSE PLAN
(a.) Conduct an Annual Inspection of the Plant Facilities.
(b.) Update the SPPP based on the Annual Review.
(c.) The Director of Public Utilities (DPU) shall have overall responsibility for
implementing the Spill Prevention and Response Plan.
(d.) The Chief Operator shall have day-to-day responsibility for operating
procedures and controlling potential contamination of stormwater.
(e.) If an accidental chemical spill results in potential stormwater contamination the
Chief Operator shall take immediate steps to protect the spill from entering the
catch basins. This shall initially consist of placing impervious protection devices
around the catch basins affected by the spill. These devices shall be on hand at
the wastewater treatment plant at all times. The Chief Operator shall then
contact the DPU for instructions on spill clean up and follow-up measures.

(f) Contact the Division of Water Quality if the accidental spill results in any water
quality violation.
(g.) A post evaluation shall be conducted after the spill cleanup has been completed
to determine the cause of the spill and to take necessary measures to adjust
operation and maintenance procedures to eliminate as much as possible the
potential for contamination to stormwater.
4. PREVENTATIVE MAINTENANCE AND GOOD HOUSEKEEPING
PROGRAM
The Town of Weldon's operation and maintenance staff at the wastewater treatment
plant consist of the Director of Public Utilities (DPU) and two operators. Both
operators at the wastewater treatment plant are responsible for conducting an
assessment of potential pollutant sources once per month. The assessment shall
consist of a walk through of the wastewater treatment plant facility documenting any
changes in the use and storage of chemicals at the wastewater treatment plant. Any
changes shall be noted and identified to the DPU who shall perform the following
actions upon notification of any changes.
I , Visually inspect the change to verify if they are the result of approved operational
procedures.
2. Identify if the changes will become permanent.
3. Review the non-structural BMPs to take immediate action as necessary to prevent
potential contamination of stormwater.
4. If needed consult the Town Engineer to modify the Stormwater Pollution
Prevention Plan as necessary to incorporate measures to prevent potential
contamination of stormwater.

5. EMPLOYEE TRAINING
The DPU shall conduct annual training on proper spill response, cleanup procedures and
preventative maintenance activities.
6. RESPONSIBLE PARTY
The DPU shall be responsible for the overall coordination, development, implementation,
and revision to the SPPP. The DUP shall utilize the Town Engineer as necessary for
review and updating components of the SPPP.
7. PLAN AMENDMENT
The Town of Weldon shall amend the SPPP whenever there is a change in design,
construction, operation, or maintenance at the wastewater treatment plant which has a
significant effect on the potential for the discharge of pollutants to the Roanoke River.
The SPPP shall be reviewed by the Town Engineer on an annual basis.
8. FACILITY INSPECTION PROGRAM
The DPU shall conduct an inspection of all stormwater systems on the following
schedule:

The inspections will be conducted either by the DPU personally, or the DPU shall have
the Town Engineer conduct the inspection. The inspections shall be documented
recording the date and time of the inspection, individual(s) performing the inspection and
a narrative description of the facility's stormwater control systems, plant equipment and
systems. Records of these inspections shall be incorporated into the SPPP and shall be
kept on file for five (5) years.
The stormwater discharge characteristic monitoring as required in Part II of the General
Stormwater Permit identified above shall be performed in addition to the facility
inspections.
9. IMPLEMENTATION
The Town of Weldon shall document all monitoring, measurements, inspections and
maintenance activities and training provided to employees, including the log of the
sampling data and of activities taken to implement BMPs. Such documentation shall be
kept on -site for a period of five (5) years and made available to the Division of Water
Quality upon request.

at 525 River Road, Weldon, NC 27890 under NPDES Permit NC0025721. On
September 23, 2005 this plant was certified by the Division of Water Quality to operate
under General Storm Water Permit No. NCG110000-
This Certification is provided by Green Engineering pursuant to Part II, Section A part 1
(e) on behalf of the Town of Weldon. The plant was inspected by Kurt Wright, PE of
Green Engineering with Donald Crowder, Director of Public Utilities and ORC of the
WWTP.
This certifies that the stormwater outfall located at the Weldon Wastewater Treatment
Plant with Lat: 360 25' 24" and Long: 77' 34' 39" has been evaluated for the presence of
non-stormwater discharges.
To the best of my knowledge and belief, no non-stormwater charges appear to be

is under review. In reality, that should have been "killed off" and NCGll

on the SP3 manual, so I think they have actually received it. Would you have time
to find the NCGll COC number in the next few days?

exposure certification. We received your no -exposure certification on August 25, 2005. Upon
further review from the Raleigh regional office, it has been determined that your facility should
not receive a no -exposure certification. I apologize for any confusion surrounding this issue.
We are denying your request for a no -exposure certification and are issuing a certificate of
coverage under the general stormwater permit for waste water treatment plants (NCG110000).
In accordance with your application for a discharge permit received on March 3, 2005 we
are forwarding herewith the subject certificate of coverage to discharge under the subject state —
NPDES general permit. This permit is issued pursuant to the requirements of North Carolina
General Statute 143-215.1 and the Memorandum of Agreement between North Carolina and the
US Environmental Protection Agency dated May 9, 1994 (or as subsequently amended).
Please take notice that this certificate of coverage is not transferable except after notice to
the Division of Water Quality. The Division of Water Quality may require modification.or
revocation and reissuance of the certificate of coverage.
This permit does not affect the legal requirements to obtain other permits which may be

treatment plants on March 4, 2005. 1 apologize for the length of time it has taken the Division to
respond to your NOI. Myr] Nisely of the Raleigh Regional Office conducted a site visit at your
facility on May 20, 2005, and he advised me that your facility would qualify for a no -exposure
certification. I am writing this letter to inform you of his comments, and I have attached the no -
exposure certification form for your review. Please inform me if you would like to receive your
stormwater permit or if you would like to certify that no -exposure conditions at your facility

GENERAL INFORMATION
This WWTP will almost certainly qualify for a No Exposure Certification. Chemicals in the
plant include caustic stored in a small building near the influent structure, chlorine and sulfur
dioxide safely stored in a brick building for disinfection, and oil for equipment inside the
maintenance building. Sludge handling has a collection drain system that returns any spillage to
the head of the plant.
The facility has stormwater collection drains strategically located throughout the plant that take
rainwater to a retention pond at the northeast corner of the site before release to the Roanoke
River.

Folks, Ken S. and I have talked this PM, and he has convinced me that we cannot
really offer No Exposure unless an entire facility is under roof. To do so, even
case by case, will get into very subjective calls and enforcement difficulties. I
was wrong in urging Don Crowder at Weldon to make this application. We should deny
it and go back to the application they made for the NCGll permit. Sorry for the
delay!

Submission of this No Exposure Certification constitutes notice that the entity identified in Section A does not require permit authorization for its storm water
discharges associated with industrial activity in the State identified in Section B under EPA's Storm Water Multi -Sector General Permit due to the existence
of a condition of no exposure.
A condition of no exposure exists at an industrial facility when all industrial materials and activities are protected by a storm resistant shelter to prevent
exposure to rain, snow, snowmelt, and/or runoff. Industrial materials or activities include, but are not limited to, material handling equipment or activities,
industrial machinery, raw materials, intermediate products, by-products, final products, or waste products. Material handling activities include the storage,
loading and unloading, transportation, or conveyance of any raw material, intermediate product, final product or waste product. A storm resistant shelter is
not required for the following industrial materials and activities:
— drums, barrels, tanks, and similar containers that are tightly sealed, provided those containers are not deteriorated and do not leak. "Sealed"
means banded or otherwise secured and without operational taps or valves;
— adequately maintained vehicles used in material handling; and
— final products, other than products that would be mobilized in storm water discharges (e.g., rock salt).
A No Exposure Certification must be provided for each facility qualifying for the no exposure exclusion. In addition, the exclusion from NPDES permitting is
available on a facility -wide basis only, not for individual outfalls. If any industrial activities or materials are or will be exposed to precipitation, the facility is
not eligible for the no exposure exclusion.
By signing and submitting this No Exposure Certification form, the entity in Section A is certifying that a condition of no exposure exists at its facility or site,
and is obligated to comply with the terms and conditions of 40 CFR 122.26(g),
ALL INFORMATION MUST BE PROVIDED ON THIS FORM.
Detailed instructions for completing this form and obtaining the no exposure exclusion are provided on pages 3 and 4.

Obtaining and Maintaining the No Exposure Exclusion
This form is used to certify that a condition of no exposure exists at the
industrial facility or site described herein. This certification is only applicable
in jurisdictions where EPA is the NPDES permitting authority and must be
re -submitted at least once every five years.
The industrial facility operator must maintain a condition of no exposure at
its facility or site in order for the no exposure exclusion to remain applicable.
If conditions change resulting in the exposure of materials and activities to
storm water, the facility operator must obtain coverage under an NPDES
storm water permit immediately.

Completing the Form
You must type or print, using uppercase letters, in appropriate areas only.
Enter only one character per space (i.e., between the marks). Abbreviate
if necessary to stay within the number of characters allowed for each item.
Use one space for breaks between words. One form must be completed
for each facility or site for which you are seeking to certify a condition of no
exposure. Additional guidance on completing this form can be accessed
through EPA's web site at www.epa.govlowm/sw. Please make sure you
have addressed all applicable questions and have made a photocopy for
your records before sending the completed form to the above address.
Section A. Facility Operator Information
1. Provide the legal name of the person, firm, public organization, or any
other entity that operates the facility or site described in this certification.
The name of the operator may or may not be the same as the name of
the facility. The operator is the legal entity that controls the facility's
operation, rather than the plant or site manager.
2. Provide the telephone number of the facility operator.
3. Provide the mailing address of the operator (P.O. Box numbers may be
used). Include the city, state, and zip code. All correspondence will
be sent to this address.
4. Indicate whether the industrial facility is operated by a department or
agency of the Federal Government (see also Section 313 of the Clean
Water Act).
5. Enter the latitude and longitude of the approximate center of the facility
or site in degrees/minutes/seconds. Latitude and longitude can
be obtained from United States Geological Survey (USGS) quadrangle
or topographic maps, by calling 1-(888) ASK-USGS, or by accessing
EPA's web site at hitp://www.epa,gov/owm/swfindustry/index.htm and
selecting Latitude and Longitude Finders under the Resources/Permit
section.
Latitude and longitude for a facility in decimal form must be converted
to degrees (1), minutes ('), and seconds (") for proper entry on
the certification form. To convert decimal latitude or longitude to
degrees/minutes/seconds, follow the steps in the following example.
Example: Convert decimal latitude 45.1234567 to degrees (°), minutes
(% and seconds (").
a) The numbers to the left of the decimal point are the degrees: 45°.
b) To obtain minutes, multiply the first four numbers to the right of the
decimal point by 0,006: 1234 x 0.006 = 7.404,
c) The numbers to the left of the decimal point in the result obtained
in (b) are the minutes: 7'.
d) To obtain seconds, multiply the remaining three numbers to the
right of the decimal from the result obtained in (b) by 0.06:
404 x 0.06 = 24.24. Since the numbers to the right of the decimal
point are not used, the result is 24".
e) The conversion for 45.1234567 = 45° 7' 24".
6. Indicate whether the facility was previously covered under an NPDES
storm water permit. If so, include the permit number.
7. Enter the 4-digit SIC code which identifies the facility's primary activity,
and second 4-digit SIC code identifying the facility's secondary activity,
if applicable. SIC codes can be obtained from the Standard Industrial
Classification Manual, 1987.
8. Enter the total size of the site associated with industrial activity in acres.
Acreage may be determined by dividing square footage by 43,560, as
demonstrated in the following example.
Example: Convert 54,450 ft2 to acres
Divide 54,450 h2 by 43,560 square feet per acre:
54,450 ft2 + 43,560 ft2/acre = 1.25 acres.
9. Check "Yes" or "No" as appropriate to indicate whether you have paved
or roofed over a formerly exposed, pervious area (i.e.. lawn, meadow,
dirt or gravel road/parking lot) in order to qualify for no exposure. If yes,
also indicate approximately how much area was paved or roofed over
and is now impervious area.

Section C. Exposure Checklist
Check "Yes" or "No" as appropriate to describe the exposure conditions at
your facility. If you answer "Yes" to ANY of the questions (1) through (11)
in this section, a potential for exposure exists at your site and you cannot
certify to a condition of no exposure. You must obtain (or already have)
coverage under an NPDES storm water permit. After obtaining permit
coverage, you can institute modifications to eliminate the potential for a
discharge of storm water exposed to industrial activity, and then certify to
a condition of no exposure.
Section D. Certification Statement
Federal statutes provide for severe penalties for submitting false information
on this application form. Federal regulations require this application to be
signed as follows:
For a corporation: by a responsible corporate officer, which means:
(i) president, secretary, treasurer, or vice-president of the corporation
in charge of a principal business function, or any other person
who performs similar policy or decision making functions for the
corporation, or
(ii) the manager of one or more manufacturing, production, or
operating facilities, provided the manager is authorized to make
management decisions which govern the operation of the
regulated facility including having the explicit or implicit duty of
making major capital investment recommendations, and initiating
and directing other comprehensive measures to assure long
term environmental compliance with environmental laws and
regulations; the manager can ensure that the necessary systems
are established or actions taken to gather complete and accurate
information for permit application requirements; and where
authority to sign documents has been assigned or delegated to
the manager in accordance with corporate procedures;
For a partnership or sole proprietorship: by a general partner or the
proprietor; or
For a municipal, State, Federal, or other public facility: by either a
principal executive or ranking elected official.

Public reporting burden for this certification is estimated to average 1.0 hour
per certification, including time for reviewing instructions, searching existing
data sources, gathering and maintaining the data needed, and completing
and reviewing the collection of information. Burden means the total time,
effort, or financial resources expended by persons to generate, maintain,
retain, or disclose to provide information to or for a Federal agency. This

We received your Notice of Intent for the NCG 110000 stormwater permit for wastewater
treatment plants on March 4, 2005. 1 apologize for the length of time it has taken the Division to
respond to your NOI. Myrl Nisely of the Raleigh Regional Office conducted a site visit at your
facility on May 20, 2005, and he advised me that your facility would qualify for a no -exposure
certification. I am writing this letter to inform you of his comments, and I have attached the no -
exposure certification form for your review. Please inform me if you would like to receive your
stormwater permit or if you would like to certify that no -exposure conditions at your facility
exist. The no -exposure form does not require any fees.

GENERAL INFORMATION
This WWTP will almost certainly qualify for a No Exposure Certification. Chemicals in the
plant include caustic stored in a small building near the influent structure, chlorine and sulfur
dioxide safely stored in a brick building for disinfection, and oil for equipment inside the
maintenance building. Sludge handling has a collection drain system that returns any spillage to
the head of the plant.
The facility has stormwater collection drains strategically located throughout the plant that take
rainwater to a retention pond at the northeast corner of the site before release to the Roanoke
River.
