Enclosed is the application packet for renewal of Franklin County's Industrial Stormwater Permit (NPDES
General Permit # NCG 110000, Certificate of Coverage # NCG110084)
The process of maintenance and treatment at the Wastewater Treatment Plant site has not changed significantly
since the issuance of the previous permit renewal. There are no new contaminants of concern being implemented
and all areas of potential runoff have been addressed accordingly. Franklin County staff have repaired driveways
and ditches leading to outfalls #1 and #2 to prevent any erosion and deposition from occurring. Silt fence has been
added to the catch basin leading to Outfall #3.

SUPPLEMENTAL INFORMATION REQUIRED FOR RENEWAL OF INDIVIDUAL
NPDES STORMWATER PERMIT
Two copies of each of the following shall accompany this submittal in order for the application
to be considered complete:
(Do not submit the site Stormwater Pollution Prevention Plan)
Initials
1. A current Site Map from the Stormwater Pollution Prevention Plan. The location of
industrial activities (including storage of materials, disposal areas, process areas and
loading and unloading areas), drainage structures, drainage areas for each outfall,
building locations and impervious surfaces should be clearly noted.
2. A summary of Analytical Monitoring results during the term of the existing permit
(if your permit required analytical sampling). Do not submit individual lab reports.
The summary can consist of a table including such items as outfall number,
parameters sampled, lab results, date sampled, and storm event data.
3. A summary of the Visual Monitoring results. Do not submit individual monitoring
reports. The summary can consist of a table including such items as outfall number,
parameters surveyed, observations, and date monitoring conducted.
4 4. A summary of the Best Management Practices utilized at the permitted facility.
Summary should consist of a short narrative description of each BMP's in place at
the facility. If the implementation of any BMP's is planned, please include
information on these BMP's.
5. A short narrative describing any significant changes in industrial activities at the
permitted facility. Significant changes could include the addition or deletion of
work processes, changes in material handling practices, changes in material storage
practices, and/or changes in the raw materials used by the facility.
6. Certification of the development and implementation of a Stormwater Pollution
Prevention Plan for the permitted facility (Sign and return attached form).
If the final year analytical monitoring of the existing permit term has not been completed
prior to filing the renewal submittal, then the last years monitoring results should be submitted
within 30 days of receipt of the laboratory reports. (i.e. do not withhold renewal submittal
waiting on lab results)

Stormwater Best Management Practices
Summary
It is the goal of Franklin County Public Utilities to ensure these BMP's are followed to prevent
pollutants from entering the Surface Waters of North Carolina. The staff of the WWTP are continually
striving to keep the runoff waters of the plant site clean and clear.
Storage — As a wastewater treatment plant we have a number of trucks, tractors and other
equipment onsite. All equipment, including; tractors, backhoes, lawnmowers and ditch witches
are store under covered or enclosed shelters when not in use.
Maintenance — Maintenance of the afore mentioned equipment is undertaken within enclosed
shop areas and remains there until all such maintenance and/or repairs are completed.
All parts, oils and greases are stored inside the maintenance buildings in their original containers
and in accordance with the manufacturer's recommendations. The use of all potential
pollutants associated with the maintenance and/or repair of equipment is completed following
all manufacturer's safety specifications and within Franklin County's material handling policies.
All maintenance personnel are trained on the proper use, storage and disposal of all potential
contaminants used onsite and in the field.
Loading and Unloading —All loading and unloading areas are clearly marked on the sitemap. No
oils or grease arrive onsite in quantities that require removal with large equipment. All potential
contaminates are hand carried or hand trucked to the maintenance buildings.
Waste Disposal — All solid waste is disposed into a dumpster and hauled away. All oils and
greases are contained in approved containers and recycled at the County's Sanitary Landfill.
Training — Our training program is administered annually or as new employees are hired to
ensure everyone understands the policies regarding stormwater runoff. This training program is
mandatory for all staff and addresses the use of potential contaminants.

Enclosed please find a copy of the Compliance Evaluation Inspection form for the stormwater inspection
conducted by Mack Wiggins and of the Raleigh Regional Office on December 12, 2012. The assistance offered
by Mr. Steve Styers, Mr. Payton Proctor and Mr. Mark Prowell was appreciated.
1. The following stormwater observations were noted:
1. The Stormwater General Permit Certificate of Coverage (NCG 110084) was issued to this facility on
June 1, 2008. The General Permit expires on May 31, 2013.
2. Franklin County Wastewater Treatment Plant has three stormwater outfalls. All outfalls were
observed during the inspection. Outfalls were assessable for sampling at the time of the inspection.
3. The facility perform vehicle maintenance activities on -site, but does not use more than 55 gallons of
new motor oil when averaged over a calendar year. Therefore, only qualitative monitoring is required.
Qualitative monitoring was conducted as required in NCG 1 10000.
4. The Stonmwater Pollution Prevention Plan has been implemented. The plan does contain most items
required by Part II Section A with the exception of the following:
I. The site map in the plan must illustrate stormwater flow direction.
2. Up -date and include the responsible party's list in Part IV of your plan.
5. All chemicals have secondary containment.

Mack Wiggins of the North Carolina Division of Water Quality, Surface Water Protection Section,
Raleigh Regional Office, conducted a stormwater compliance evaluation inspection of the Franklin
County Wastewater Treatment Plant on December 28, 2009. The assistance and cooperation Mr. Steve
Styers was very helpful and appreciated during the inspection process.
1.The following observations were made during the stormwater inspection:
1. The Stormwater Pollution Prevention Plan for this facility contained all the components as
required by NCGI 10000. The facility must continue to aggressively implement the plan.
2. Qualitative monitoring was performed at the stormwater outfalls as required by NCGI 10000.
Continue to monitor your outfalls. Qualitative monitoring must be conducted in accordance
with the schedules in Part I I, Section B, Table I (Monitoring Requirements), Table 2
(Monitoring Schedule).
3. Analytical monitoring is not required at this facility. The facility does not do any vehicle
maintenance.
4. All chemicals are stored in the secondary containment as required.
The facility has three outfalls. All outfalls were accessible for sampling during the time of
the inspection. Outfalls were not discharging during the time of the inspection.
6. In relation to the Stormwater Pollution Prevention Plan, the plan does include and address.an
inspection program including; Preventive Maintenance and Good Housekeeping. We
recommend that the facility create an inspection form for the facilities inspection programs.

On August 26, 2008, Vicki Webb of the Raleigh Regional Office conducted a compliance evaluation inspection
of the subject facility. The assistance of Steven Styers, operator in responsible charge (ORC), and Mark
Prowell, backup operator in responsible charge (BORC), was appreciated as it facilitated the inspection process.
The following observations were made:
The Franklin County WWTP is permitted to discharge at a rate of 3 MGD and consists of: influent pump
station with both mechanical and manual bar screens, grit chamber, influent flow measurement, two aeration
basins, two secondary clarifiers, an aerobic sludge digester, two tertiary filters, dual train ultraviolet
disinfection, a Parshall flume, effluent flow measurement, post aeration and a backup power generator. The
facility discharges to the receiving waters designated as the Cedar Creek, a class C, nutrient sensitive water
(NSW), in the Tar Pamlico river basin.
A review of the Discharge Monitoring Report (DMR) data for the 12-month period from June 2007 through
July 2008 was performed prior to this inspection. The staff of this facility should be commended for not
having any permit limit or monitoring violations in this 12-month time period.
DMRs for January 2008 and June 2008 were reviewed and compared to lab reports. No mistakes or
inconsistencies were noted. Chains of custody were complete and kept on file as required. The annual
compliance report was reviewed and found to be complete and acceptable.
The plant and laboratory was very clean and well maintained. No operational or maintenance issues were
noted. Visitation and operations logs were reviewed and found to be complete and satisfactory. The plant
effluent was free and clear of solids or any other pollutant indicators. The receiving stream looked healthy
and showed no adverse conditions and right-of-way (ROW) to the outfall was well maintained.

A compliance evaluation inspection was also conducted of the facility's Stormwater permit NCG110084
during the August 26th visit. All of the facility's stormwater outfalls were observed during the inspection.
The ROW to the outfalls was well maintained.
The stormwater pollution prevention plan (SP3) was reviewed. It contained all the required components and
met all requirements. Qualitative and quantitative inspection records were available for review and found to
be complete and acceptable.
The overall conditions of the Franklin County WWTP are compliant with Division standards. If you have

I conducted a compliance evaluation inspection of the Franklin County regional wastewater treatment plant
(WWTP) on 12 July 2007. The cooperation and assistance of Mr. Steven Styers, operator in responsible charge
(ORC) was greatly appreciated. Inspection findings are summarized below and an inspection checklist is attached
for your records.
Franklin County's WWTP discharges into receiving waters known as Cedar Creek, a class C, nutrient sensitive
water (NSW), in the Tar Pamlico river basin. The plant consists of an influent pump station with both mechanical
and manual bar screens, a grit chamber, influent flow measurement, two aeration basins, two secondary clarifiers,
an aerobic sludge digester, two tertiary filters, dual train ultraviolet disinfection, a Parshall flume, effluent flow
measurement, post aeration and a backup power generator.
The plant was very clean and well maintained. No operational/maintenance issues were noted. Visitation and
operations logs were reviewed and found to be complete and satisfactory. All plant components were operational
except the influent flow recorder, which had recently been struck by lightning and was scheduled for replacement;
the effluent flow meter is used for reporting. Only half the plant was in operation because flows are currently low
enough that half of the plant provides more than enough treatment capacity. The plant effluent was clear and free
of solids or any other pollutant indicators. The receiving stream looked healthy and showed no adverse conditions
and the right-of-way to the outfall was well maintained.
Discharge monitoring reports (DMR) for November 2006 and March 2007 were reviewed and compared to lab
reports. No mistakes or inconsistencies were noted. Chains of custody were complete and kept on file as required.
DMRs on file at the regional office for the previous year revealed no permit limit violations. The annual
compliance report was reviewed and found to be complete and acceptable.
The laboratory was clean and well maintained. Wrenn Scofield operates the laboratory. The coliform incubator
was set at 44.5 degrees Celcius. Laboratory records were satisfactory and complete. Composite samplers were
within the required temperature range and no problems were noted.
The stormwater pollution prevention plan (SP3) was reviewed. It contained all the required components and met
all requirements except for the annual review. Although the operators said it was reviewed more than annually,
there was no record of it. Please have operators document their reviews of the SP3 and any changes to their
program that they find necessary to keep it up-to-date and within requirements.
