Application Form. The application must he completed and returned by March 4. 2002 in order to assure continued
coverage under the general permit.
Failure to request renewal within this time period may result in a civil assessment of at least $250.00. Larger

I visited your office on January 2, 1997 to investigate an oil spill complaint. During this
investigation we discussed the possibility of installing an oil/water separator on your property to prevent
discharges of oil into surface waters of the state.
This office has reviewed your request for a permit application to discharge stormwater and has
determined that your facility and type of industrial activities would be covered under General Permit No.
NCGO8000, which would allow you to discharge stormwater under the National Pollutant Discharge
Elimination System. Enclosed is a Notice of Intent requesting coverage under General Permit No.
NCG080000, which covers the industrial activities located at your facility. I have also enclosed a copy
of General Permit No. NCGO80000 for your review.
The Notice of Intent must be completed with all of the necessary information and submitted with
a permit filing fee of $400.00 to the following address:
