The attached documentation includes the following: 1. In response to the lid of our cardboard recycling container being opened, I have reviewed this with the business manager at the facility and informed him that the lids to the trash and cardboard container should be closed at all times. This will also be reviewed with our housekeeping crew immediately. 2. In response to annual review of the SP3, we currently have an annual SP3 Compliance Inspection Report. The review was completed on January 24, 2007; however the form was not in the SP3 binder that is kept at the facility. It was in our SP3 binder that we keep in our district office in Raleigh. A copy of this evaluation was sent to the center and placed in their binder on 05/31/07, A review of the SP3 will be conducted annually and documentation of that review kept in the facilities binder for five years. 3. In response to the general location map not showing the latitude and longitude for each of the discharge points, I currently have a consultant contracted to perform semi-annual site inspections at each of our facilities. They will be obtaining the latitude and longitude data for each of the discharge points. I LS 1627 Navaho Drive !' In response to the missing annual analytical & qualitative monitoring data from 1994, it is my understanding that we have to keep 5 years worth of data; therefore, we should have 2002 — present. We did have the analytical information from 2002 & 2006 (the analytical thresholds were not exceeded during these samplings therefore annual analysis was not required, only the first year, 2002 and the last year 2006). As far as the qualitative monitoring data, there has been an inadvertent oversight in maintaining documentation for three years instead of five in our district office. We are modifying the location of the records as well as the retention schedule to reflect this change and expect to have the modification completed no later than July 1, 2007. I conducted a compliance evaluation inspection (CEI) of the subject facility on 15 May 2007. Your cooperation and assistance was helpful and appreciated. Inspection results are summarized below. 2. This facility discharges industrial stormwater from stormwater discharge outfalls 001 and 002 into an unnamed tributary to Compass Creek, a class C, nutrient sensitive water in the Tar -Pamlico river basin. The site was generally clean and well maintained. There were no discharges at the time of the inspection. 3. Stormwater pollution risks are limited at the site. There is a vehicle fueling station located on the west side of the facility that includes one underground diesel storage tank. There is no secondary containment for fuel spills but spill kits are kept in the immediate vicinity of the fueling station. The facility also has two dumpsters, one of which was not covered at the time of the inspection. Please remind employees to keep the dumpsters covered at all times in the future. 4. All vehicle maintenance, including vehicle washing, is accomplished inside the facility. The drainage from these activities goes through sand traps and an oil/water separator (OWS). All these discharges have been piped into the sanitary sewer. 5. The site has a separate, stand-alone building located on the north end of the facility, in which hazardous wastes are stored prior to being picked up for disposal. Wastes are in sealed and labeled containers. Wastes inside the building are unexposed. 6. The stormwater pollution prevention plan (SP3) was evaluated. The SP3 included most of the required components and each appears to have been implemented effectively. Employee training, maintenance, housekeeping, etc., are being done as required. Please note that all records required by the SP3 must be kept on site with the SP3 for a period of five years. 7. Please reply to our office within 30 days of your receipt of this letter. Your reply should include a corrective action plan and implementation schedule for those issues noted above in paragraphs 3 and 6a — 6c. You are reminded that the issues above constitute violations of your permit and NC General Statute 143-215.1, and are subject to civil penalties of up to $25,000.00 per day per violation. This permit expires on August 31, 2002, The Division staff is currently in the process of rewriting this permit and is scheduled to have the permit reissued by late summer of 2002. Once the permit is reissued, your facility would be eligible for continued coverage under the reissued permit. In order to assure your continued coverage under the general permit, you must apply to the Division of Water Quality (DWQ) for renewal of your permit coverage. To make this renewal process easier, we are informing you in advance that your permit will be expiring. Enclosed you will find a General Permit Coverage Renewal Application Form. The application must be completed and returned by March 4, 2002 in order to assure continued coverage under the general permit. Failure to request renewal within this time period may result in a civil assessment of at least $250.00. Larger penalties may be assessed depending on the delinquency of the request. Discharge of stormwater from your facility without coverage under a valid stormwater NPDES permit would constitute a violation of NCGS 143-215.1 and could result in assessments of civil penalties of up to $10.000 per day. Please note that recent federal legislation has extended the "no exposure exclusion" to all operators of industrial facilities in any of the 1 1 categories of "storm water discharges associated with industrial activity," (except construction activities).
