the Memorandum of Agreement between North Carolina and the U.S. Environmental
Protection Agency.

standards and regulations promulgated and adopted by the North Carolina Environmental
Management Commission, and the Federal Water Pollution Control Act, as amended,
Mann + Hummel Filtration Technology
is hereby authorized to discharge stormwater from a facility located at:

to receiving waters designated as Catawba Creek, a class C water in the Catawba River Basin; in
accordance with the effluent limitations, monitoring requirements, and other conditions set forth

This certificate of coverage shall become effective July 5, 2016.
This Certificate of Coverage shall remain in effect for the duration of the General Permit.

We have received a letter dated March 2, 2015 from Mr. Tim McCorkle, Project
Manager, with ARM Environmental Services, Inc. on your behalf, requesting that your
facility be considered for semi-annual sampling events. An inspection of the facility was
conducted by Mr. Samar Bou-Ghazale on March 19, 2015. File review of the facility
confirmed that the sample collected for zinc and copper on January 23, 2015 were below
the bench mark value. Please note that in order to revert to the semi-annual sampling it is
required by the permit that you have to pass 3 consecutive sampling events.

Thank you for the submitted information regarding Zinc/Copper exceeding the benchmark value at the
subject facility.
As requested a site investigation will be performed at the subject facility and if it is determined that
exceeding the bench mark values are beyond your control and that you have exhausted all the
possibilities to be in compliance with the benchmark, then your facility might be considered for the
requirements mentioned in Tier three. In the meantime it is requested that you conduct an upstream
and downstream sample in order for us to better evaluate the migration of contaminants from the site.

Comment: Facility applied for a permit renewal in 2007 and retained a certified receipt
to prove it. Since they did not receive the new permit, they stopped all monitoring. In
2010 the facility started again on their own collecting data according to old permit.
Facility contacted Raleigh after being questioned by the Regional Office and started the
process to renew the permit.

Monitoring Report and the Qualitative Monitoring Report for Wix Filtration Products
Division — Allen Plant. This report is being submitted to fulfill the sampling provisions
of General Permit No. NCG030000 (COC No. NCG030412) with an expiration date of
August 31, 2007. Stormwater sampling was taken on 2/25/07 to complete all of the
requirements (TTO requirements) for the last year of Stormwater permit. As soon as we
get those results we will forward those onto the Division of Water Quality.

Your facility is currently covered for stormwater'discharge under General Permit NCG030000. This permit expires
on August 31. 2002. The Division staff is currently in the process of rewriting this perrnit and is scheduled to have
the permit reissued by late summer ol'2002. Once the permit is reissued, your facility would be eligible for
continued coverage under the reissued permit.
]n order to assure your continued coverage under the general permit, you must apply to the Division of Water
Quality (DWQ) for renewal ol'your permit coverage. To make this renewal process easier, we are informing you in
advance that your permit will he expiring. Enclosed you will find a General Permit Coverage Renewal
Application Form. The application must he completed and returned by March 4, 2002 in order to assure continued
coverage under the general permit.
Failure to request renewal within this time period may result in a civil assessment of at ]east $250.00. Larger
penalties may he assessed depending on the delinquency of the request. Discharge of stormwater from your facility
without coverage under a valid stormwater NPDES permit would constitute a violation ol' NCGS 143-215.1 and
could result in assessments of civil penalties of up to $10.000 per day.
Please note that recent federal legislation has extended the "no exposure exclusion" to all operators of industrial
facilities in any of the 1 I categories of "storm water discharges associated with industrial activity," (except
construction activities). If you feel your facility can certify a condition of "no exposure", i.e. the facilty industrial
materials and operations are not exposed to stormwater, you can apply for the no exposure exclusion. For additional

If the subject stormwater discharge to waters of the state has been terminated, please complete the enclosed
Rescission Rerluest Form. Mailing instructions are listed on the bottom of the form. You will be notified when the
rescission process has been completed.

Re: Wix Facilities - Stormwater Sampling
Dear Ms. Tow:
Because there has been no storm event meeting the necessary stormwater sampling criteria at -a
time feasible for a sampling by one of our engineers, the Wix facilities in(Gasionia have not yet
met their stormwater sampling requirements.
I am writing to inform you that we are still attempting to take the necessary samples when the
weather is acceptable for such, as well as looking into other options including training on -site
employees to take the samples.

Wix Corporation - Allen Plant

The Division of Water Quality (Division) has reviewed your petition submitted on behalf of the
Wix Corporation for representative outfall status at their Allen Plant facility located at 2900
Northwest Boulevard, Gastonia, North Carolina.
You petitioned the Division to allow stormwater discharge outfall #1 (SDO #1) to be
representative of outfall #2 (SDO #2). Based on a review of the submitted documentation, it is
clear to the Division that SDO #1 would contain the same types of potential pollutants as SDO #2.
Therefore, the Division agrees with your petition and grants your request to sample SDO #1 as
representative of SDO #2.
Please note that qualitative monitoring is still required at all outfalls at the facility regardless of
representative outfall status as stated in Part 11, Section C, Paragraph 1 of General Permit
NCG030000,

200,000 gallon tank in drainage area 2 contains water for fire suppression system.
Non -contact cooling water discharge is covered under NCG500185
Trash compactor in drainage area l (Outfall to be sampled)
Summary - Both drainage areas contain similar potential sources of pollutants, oil and
grease from vehicular traffic and pallet / metal bin storage exposure. Request was to
sample outfall 1 as representative, which seems reasonable.
Ozark Plant (NCG030413)
Trench drain near trash compactor and dumpster would be excluded if request was
granted as submitted (Drainage areas 1 & 2).
No bulk storage of liquids.
Summary - Four outfall identified. Outfalls 1 & 2 having similar exposures (Roof,
vehicles, compactor, and dumpster) and 3 & 4 having similar exposures (Roof, vehicles,
metals waste bin). Recommend modifying request to include sampling of outfall 2 in order
to monitor trench drain effluent.
Dixon Plant (NCG030414)
No above ground bulk storage of liquids, one UST indicated
Drainage area 1 exposure primarily vehicular, some shipping exposure.
Summary - Drainage area 2 contains most of the potential sources of exposure, dumpsters,
metal bins, trash compactor, vehicular, and shipping. Request was to sample outfall 2 as
representative, which seems reasonable.

In accordance with your application for discharge permit received on November 13, 1996, we are
forwarding herewith the subject certificate of coverage to discharge under the subject state - NPDES general
permit. This permit is issued pursuant to the requirements of North Carolina General Statute 143-215 .1 and the
Memorandum of Agreement between North Carolina and the US Environmental Protection agency dated
December 6, I983.
If any parts, measurement frequencies or sampling requirements contained in this permit are
unacceptable to you, you have the right to request an individual permit by submitting an individual permit
application. Unless such demand is made, this certificate of coverage shall be final and binding.
Please take notice that this certificate of coverage is not transferable except after notice to the Division
of Water Quality. The Division of Water Quality may require modification or revocation and reissuance of the
certificate of coverage.
This permit does not affect the legal requirements to obtain other permits which may be required by the
Division of Water Quality or permits required by the Division of Land Resources, Coastal Area Management
Act or any other Federal or Local governmental permit that may be required.
