Enclosed is a copy of the Compliance Evaluation Inspection Report for the inspection conducted at the
subject facility on April 2, 2013 by Mr. Wes Bell of this Office.
This report is being issued as a Notice of Violation (NOV) due to the facility's failures to implement the
Stormwater Pollution Prevention Plan and perform the required qualitative and analytical monitoring, all violations
of the subject NPDES Stormwater Permit and North Carolina General Statute (G.S_) 143-215.1, as detailed in the
Stormwater Pollution Prevention Plan/Summary, Qualitative Monitoring, and Analytical Monitoring
Sections of the attached report. Pursuant to G.S. 143-215.6A, a civil penalty of not more than twenty-five thousand
dollars ($25,000.00) per violation, per day, may be assessed against any person who violates or fails to act in
accordance with the terms, conditions, or requirements of any permit issued pursuant to G.S. 143-2IS. 1.
It is requested that a written response be submitted to this Office by May 2, 2013, addressing the above -
noted violations in the attached report. In responding, please address your comments to the attention of Mrs.

Inspection Summary:
STORMWATER POLLUTION PREVENTION PLAN (SPPP) cont'd:
Evidence of significant erosion was observed in the drainage area and stormwater outfall during the inspection. The
facility staff should incorporate the appropriate erosion control measures to prevent/minimize the runoff of sediments from
entering the stormwater discharges and receiving stream. These control measures should also be documented in the
SPPP as Best Management Practices (BMPs).
Several small oil stains °mere observed on the ground. The facility staff must ensure that all affected areas are properly
cleaned up and disposed as part of the SPPP's Spill Response Procedures. Please note that the failure to clean up oil
discharges can subject the responsible party to potential civil penalties. in addition, spill pruveritiurt and response should
also be addressed during the annual employee training event

Commeht: The facility had contracted a company to develop a Stormwater Pollution
Prevention Plan (SPPP) several years ago; however, the SPPP did not include all of the
requirements noted in the Permit. In addition, the SPPP had not been implemented to
include employee training (once per year), three year history of significant spills or a
notation that no spills occurred (if applicable), overall SPPP review and update (once
per year), facility stormwater inspections (twice per year),qualitativelvisual monitoring .
(twice per year), analytical monitoring (twice per year), and re-evaluations of each
stormwater outfall for the presence of non-stormwater discharges (once per year). See
"Summary" Section for additional comments.
