Enclosed please find a copy of the Compliance Evaluation Inspection Report for the inspection
conducted at the subject facility on June 8, 2012 by Ms. Donna Hood of this Office.
At the time of the inspection the facility had not certified the outfalls for the absence of non-
stormwater discharges. Although significant amount of work had been performed that showed there
were no illicit discharges, it was not documented in the stormwater pollution prevention plan. Please
correct this immediately, as it was noted in the previous inspection.

gate valves that can be used to contain any onsite release. These valves should be
exercised fully to ensure they will close completely in the event that they are needed to
contain a spill. This should be part of the preventative maintenance schedule at the

Comment: The facility has designated one stormwater outfall; however, the inspection
verified that a second outfall should also be designated for the drainage area that

Quality (DWQ) for renewal of your permit coverage. To make this renewal process easier, we are informing you in

Application Form. The application must be completed and returned by October 2, 2002 in order to assure
