Thank you for submitting the data of exceedance report dated March 24, 2017, and received in this office on
March 30, 2017 for metals for the subject facility.
Please note that if you have completed tier one and two per the permit requirements and you have maintained
the tier responses in the Stormwater Pollution Prevention Plan (SWPPP) and you have exceeded the benchmark
in two consecutive months which added to four (4) consecutive sampling events above the bench mark values,
then you can submit a letter to this office requesting a site evaluation. A site evaluation will be performed and
if it is determined that you have exhausted all the possibilities to be in compliance with the benchmark and
exceeding the bench mark values are beyond your control, then your facility might be considered for the
requirements mentioned in Tier three.

Dear Stormwater Permitting Unit:
Attached is a slight revision of the Kings Mountain International, Inc. (Cleveland County, NC),
Figure 2 showing the drainage discharge from the area that contains the drums stored on the
concrete pad behind the plant. The figure also shows the storm drainage leaders and gutters and
the storm drainage inlets and pipes that discharge to the Storm Drainage outlets, as requested by
Mr. Samar Bou-Ghazale in his recent visit and confirmed in a letter from Zahid S. Khan on
October 28, 2015.
The Kings Mountain International, Inc. (KMI) Site is covered under North Carolina General
Stormwater Permit NCG0030000. Currently, KMI collects and tests stormwater discharged
from outfalls SDO1 and SD02 on a monthly basis in accordance with Tier 3 requirements.
Based on discussions with Mr. Samar Bou-Ghazale at the North Carolina Department of
Environmental Quality (NCDEQ) Mooresville Regional Office, KMI proposes to sample SD05
in place of SDO 1 and SD02.
SD05 discharges stormwater run-off collected from the site industrial activity area and provides
a more representative sample than SDOI and SD02, which discharge stormwater run-off from
the plant's roof downspouts. Please let us know if you have any questions or need any additional
information in order to grant this request.

Dear Mr. Wagenknight:
Enclosed is a copy of the Compliance Evaluation Inspection (CEI) report for the
inspection conducted at the subject facility on June 19, 2013, by Ms. Barbara Sifford of this
Office. The site drains into Dixon Branch in the First Broad River Basin. The site visit and file
review revealed that the subject facility is covered by NPDES Stormwater General Permit
NCG030000. The assistance of yourself and Mr. Ivan Copper was greatly appreciated during the
inspection.
Accordingly, the following observations were noted during the inspection:
1) Stormwater Poilution_Prevention Plan (SPPP) Yes X No
A SPPP has been developed and is updated annually. The review form needs a signature.
2) Quallitative Monitoring Yes X_ No
Qualitative monitoring has been conducted and is recorded in accordance with permit

Additional metals parameters were added in 2012 with the reissuance of the NCG030000
permits. It has been brought to the attention of the Division of Water Quality that .many
facilities are exceeding the benchmark concentrations listed in the permit. Tier One requires
the possible causes of the benchmark value exceedances to be addressed. If, after a detailed
evaluation of the facility the sources are identified as uncontrollable (i.e. building material or
fencing) and not related to the manufacturing process, then you may submit this information
to the regional office for review and a determination will be made as to whether a variance
can be issued.
The enclosed report should be self-explanatory. Overall the facility plan was well

Comment: High zinc levels in first analytical this permit cycle. May be due to scrape
materials around the plant left from previous operations. Company plans to sell this
scrape, and paint some existing metal roofs that are rusting and then see if this lowers
the concentrations. Selective isolation sampling may be done to determine if this is
from the building structure or the manufacturing operation activities. After data is
collected this should be submitted to the regional office for review to determine
compliance with benchmark concentrations.

In response to your renewal application for continued coverage under stormwater General Permit NCG030000
the Division of Water Quality (DWQ) is forwarding herewith the reissued General Permit. This permit is
reissued pursuant to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of
Agreement between the state of North Carolina and the U.S. Environmental Protection Agency, dated October
15, 2007 (or as subsequently amended).
The following information is included with your permit package:
• A new Certificate of Coverage (COC)
• A copy of General Permit NCG030000
• A copy of the Technical Bulletin for the General Permit
• Two copies of the Discharge Monitoring Report (DMR) Form
• Two copies of the Qualitative Monitoring Report Form
The General Permit authorizes discharges of stormwater, and it specifies your obligations for discharge
controls, management, monitoring, and record keeping. Please review the new permit to familiarize yourself
with all the changes in the reissued permit. Your facility has six (6) months from the time of receipt of the
permit to update your current SPPP to reflect all new permit requirements.
The first sample period of your permit begins January 1, 2013. Your facility must sample a "measureable
storm event" beginning during the periods beginning January 1 and July 1 of every year (or, if applicable,
report "No Flow," as outlined in Part Ili, Section E). Also, please note that Tier 3 Actions in Part II of your
permit are triggered by benchmark exceedances on four occasions beginning an the effective date of this
permit and do not count prior exceedances.
The more significant changes in the General Permit since your last COC was issued are noted either in the
Draft Permit Fact Sheet that accompanied the public notice (http://portal.ncdenr.org/web/wq/ws/su/current-
notices), or in the Response to Comments / Summary of Changes and Technical Bulletin documents that are
posted on the stormwater Permitting Unit's website with the new General Permit. Please visit

Some of the changes include:
Part II:
• Section A: The Stormwater Pollution Prevention Plan (SPPP) section, if applicable, has been updated
to the most current language of our permits. Additional conditions for specific industry sectors have
been added to the SPPP requirements in some cases.
• Sections 8, C: Failure to perform analytical stormwater monitoring may result in the Division requiring
that the permittee begin a monthly sampling scheme.
• Sections 8, C. A lower TSS benchmark of 50 mg/I for HQW, ORW, PNA and Tr Waters applies to these
more sensitive waters.
• Sections 8, C. The monitoring parameter Oil & Grease (O&G) has been replaced by the parameter
Total Petroleum Hydrocarbons (TPH) for vehicle maintenance areas, and in some cases, other
analytical monitoring requirements.
• Sections 8, C, D: Inability to sample due to adverse weather must be recorded in the SPPP, or in
separate on -site records if your General Permit does not require an SPPP. Adverse weather is defined
in the "Definitions" section of the permit.
• Sections 8, C. The term "Representative Storm Event" has been replaced by "Measurable Storm
Event." A measurable storm event is defined in the permit.
• Section D: If the permittee fails to respond effectively to problems identified by qualitative
monitoring, DWQ may require the permittee to perform corrective action.
Please review Parts III and IV to understand the Standard Conditions of your new NPDES General Permit,
including Compliance and Liability, Reporting, Monitoring and Records requirements; Operation and
Maintenance obligations; and Definitions. Please note that all samples analyzed in accordance with the terms
of this permit must be submitted to the Division on Discharge Monitoring Report (DMR) forms available on
the Stormwater Permitting Unit's website above. DMR forms must be delivered to the Division no later than
30 days from the date the facility receives the sampling results from the laboratory. Also note that existing
permittees do not need to submit a renewal reeuest prior to expiration unless directed by the Division.
Your coverage under the General Permit is transferable only through the specific action of DWQ. This permit
does not affect the legal requirements to obtain other permits which may be required by DENR, nor does it
relieve the permittee from responsibility for compliance with any other applicable federal, state, or local law,
rule, standard, ordinance, order, judgment, or decree. If you have any questions regarding this permit
