We recently discovered that your original application for coverage under the General Stormwater Permit for
Construction Activities incorrectly indicated the project was NOT subject to the NC Sediment Pollution Control
Act. This resulted in a Certificate of Coverage for general permit NCG250000 (which is not available yet), instead of
NCG010000, which is what applies to this project. You can see that the COC attached has an NC Reference No. thal
begins with "NCG25-"instead of
NCGO1-^
Unfortunately atthe time of review, the wrong selection for question 16a. on the eN(]| was overlooked. The answer
should have been YES toquestion 1Ga.

1) Request a new COC by filling out the eNOl application form at https://edocs.deg.nc.gov/Forms/NCGOI-NO1
2) Use the attached original documents to upload (E&SC approval and Signed Certification). If you need to view
your original NOI for other information, please go to this file link: NCC190266 COC File Be sure to keep the
"Yes" selected for question 16a.
3) Once accepted as complete, you'll receive an e-mail confirmation. That e-mail will have payment instructions,
which you can disregard. The application fee will not apply for this re -submittal because this was our
oversight, and your original application was submitted prior to this fee requirement.
4) After receiving the correct COC for this project, submit a Notice of Termination (NOT) request for the previous
COC (No. NCC190266). That NOT request is at https://edocs.deg. nc.gov/Forms/NCGOI-Termi nation
help resolve it. It will ensure that the data we send to EPA later this year is accurate for your project.
