A PCN application was submitted for this project on February 25, 2019. The application called for 92 linear feet of permanent impacts and 50 linear feet of temporary impacts to an
unnamed tributary to Blood Creek for a road crossing which would provide access to a proposed athletic fields facility. A culvert consisting of three pipes has been designed for the 25 -
year storm event at the stream crossing.

Expand the recreational facilities at Chatham Charter School and provide an access road from the existing school facilities.
4i. Describe the overall project in detail, including indirect impacts and the type of equipment to be used:*
The proposed project consists of new recreation field facilities that include an amenity building, drop-off, and parking area with an access road connecting to the existing school
facilities. The amenity building is not intended to include water or sewer connections. The paved access road will be approximately 550 linear feet in length. The access road will require
a culvert crossing of the unnamed tributary to Blood Run Creek. A culvert consisting of three pipes has been designed for the 25 -year storm event at the stream crossing. Erosion
control measures have been designed for the 10 -year storm event. Approximately 14.2 acres will be disturbed for the project. The project will be built by a contractor using typical
construction equipment including excavator, bulldozer and dump truck.
As of August 20, 2019, the stream crossing had been installed and grading had been completed on the athletic fields portion of the property. The construction of the northernmost
sediment basin (see Sheets C-103, C-104) resulted in approximately 0.03 -acres of temporary impacts as fill to Wetland A. The basin will be removed and the wetland area restored to
original grades and stabilized with a native NC wetland seed mix.

fall out prior to discharge toward the stream.

Location description: The site consists of 71.63 acres located at 2200 Hamp Stone Road, northeast of Sante Fe Circle, northwest
of Fontana Circle approximately 1 mile northwest of US Highway 64, in Siler City, NC.
Description of projects area and activity: This verification authorizes the discharge of fill material into 72 linear feet of stream
channel associated with culvert installation 20 linear feet of stream impacts for the installation of the rip rap energy dissipater
pad and 50 linear feet of temporary stream channel impacts associated with cofferdam installation and dewatering activities
associated with the culvert's installation. These impacts are associated the road crossing to access the Recreation Fields
Expansion.

the permittee to a stop work order, a restoration order, a Class I administrative penalty, and/or appropriate legal action.
This verification will remain valid until the expiration date identified below unless the nationwide authorization is modified, suspended
or revoked. If, prior to the expiration date identified below, the nationwide permit authorization is reissued and/or modified, this
verification will remain valid until the expiration date identified below, provided it complies with all requirements of the modified
nationwide permit. If the nationwide permit authorization expires or is suspended, revoked, or is modified, such that the activity would
no longer comply with the terms and conditions of the nationwide permit, activities which have commenced (i.e., are under construction)
or are under contract to commence in reliance upon the nationwide permit, will remain authorized provided the activity is completed
within twelve months of the date of the nationwide permit's expiration, modification or revocation, unless discretionary authority has
been exercised on a case-by-case basis to modify, suspend or revoke the authorization.

NOTES: NCDEQ GENERAL CONSTRUCTION CONDITIONS
1. THE CONTRACTOR SHALL MINIMIZE CLEARING WITHIN WETLANDS AND ACROSS
STREAMS TO THE GREATEST EXTENT PRACTICABLE AND SHALL NOT EXCEED
50' WIDE FOR GAS UTILITY LINES AND 40' WIDE FOR ALL OTHER UTILITY
LINES. EXCEPTIONS TO THIS CONDITION WILL REQUIRE WRITTEN APPROVAL
FROM THE PROJECT ENGINEER. FOR CONSTRUCTION CORRIDORS IN
WETLANDS AND ACROSS STREAM CHANNELS, STUMPS SHALL BE GRUBBED
ONLY AS NEEDED TO INSTALL THE UTILITY AND REMAINING STUMPS SHALL
BE CUT OFF AT GRADE LEVEL. THE GENERAL STRIPPING OF TOPSOIL WITHIN

DISPOSAL AREA. THE MIXING OF TOPSOIL AND SUBSOILS WITHIN WETLANDS
ALONG UTILITY CORRIDORS SHALL BE MINIMIZED TO THE GREATEST EXTENT
PRACTICABLE DURING EXCAVATION. THE SOILS SHALL BE PLACED ON FABRIC
TO MINIMIZE IMPACTS WHENEVER POSSIBLE. TOPSOIL EXCAVATED FROM
UTILITY TRENCHES SHALL BE PILED SEPARATELY FROM SUBSOILS AND
BACKFILLED INTO THE TRENCH ONLY AFTER THE SUBSOILS HAVE BEEN
PLACED AND COMPACTED,
4. NO WASTE, SPOIL, SOLIDS, OR FILL OF ANY KIND CAN OCCUR IN WETLANDS
OR WATERS BEYOND THE FOOTPRINT OF THE IMPACT AREA (INCLUDING
TEMPORARY IMPACTS) OR BEYOND THE PERMITTED THRESHOLDS WITHOUT
WRITTEN AUTHORIZATION FROM THE PROJECT ENGINEER. NO REMOVAL OF
VEGETATION OR OTHER IMPACTS OF ANY KIND CAN OCCUR TO STATE
REGULATED RIPARIAN BUFFERS BEYOND THE FOOTPRINT OF IMPACTS

PERMANENT IMPACTS AUTHORIZED UNDER THE PERMITS. EXCEPTIONS TO
THIS CONDITION REQUIRE WRITTEN APPROVAL FROM THE PROJECT ENGINEER.
7. EROSION CONTROL MATTING THAT INCORPORATES PLASTIC MESH AND/OR
PLASTIC TWINE SHALL NOT BE USED ALONG STREAMBANKS OR WITHIN
WETLANDS. EXCEPTIONS TO THIS CONDITION REQUIRE WRITTEN APPROVAL
FROM THE PROJECT ENGINEER.
B. AN NPDES CONSTRUCTION STORMWATER PERMIT (NCGOIOOOO)IS REQUIRED
FOR CONSTRUCTION PROJECTS THAT DISTURB ONE OR MORE ACRES OF
LAND. THE NCGO10000 PERMIT ALLOWS STORMWATER TO BE DISCHARGED
DURING LAND DISTURBING CONSTRUCTION ACTIVITIES AS STIPULATED IN THE
CONDITIONS OF THE PERMIT. IF THE PROJECT IS COVERED BY THE PERMITS,

MOVEMENT DURING LOW FLOWS. THE DIMENSION, PATTERN, AND PROFILE OF
THE STREAM ABOVE AND BELOW A PIPE OR CULVERT SHALL NOT BE
MODIFIED BY WIDENING THE STREAM CHANNEL OR BY REDUCING THE DEPTH
OF THE STREAM IN CONNECTION WITH THE CONSTRUCTION ACTIVITY.
11. APPLICATION OF FERTILIZER TO ESTABLISH PLANTED/SEEDED VEGETATION
WITHIN DISTURBED RIPARIAN AREAS SHALL COMPLY WITH ALL OTHER
FEDERAL, STATE AND LOCAL REGULATIONS. FERTILIZER APPLICATION SHALL
BE ACCOMPLISHED IN A MANNER THAT MINIMIZES THE RISK OF CONTACT
BETWEEN THE FERTILIZER AND SURFACE WATERS.
12. IF CONCRETE IS USED DURING CONSTRUCTION, THEN ALL NECESSARY
MEASURES SHALL BE TAKEN TO PREVENT DIRECT CONTACT BETWEEN
UNCURED OR CURING CONCRETE AND WATERS OF THE US. WATER THAT
INADVERTENTLY CONTACTS UNCURED CONCRETE SHALL NOT BE DISCHARGED
TO WATERS OF THE US.
13. ALL PROPOSED AND APPROVED TEMPORARY FILL AND CULVERTS SHALL BE
REMOVED AND THE IMPACTED AREA RETURNED TO NATURAL CONDITIONS
WITHIN 60 CALENDAR DAYS AFTER THE TEMPORARY IMPACT IS NO LONGER
NECESSARY. THE IMPACTED AREAS SHALL BE RETURNED TO ORIGINAL
GRADE, INCLUDING EACH STREAM'S ORIGINAL CROSS SECTIONAL DIMENSIONS,
PLANFORM PATTERN, AND LONGITUDINAL BED PROFILE. ALL TEMPORARY
IMPACTS SHALL BE RESTORED AND STABILIZED WITH NATIVE VEGETATION.
14. ALL PROPOSED AND APPROVED TEMPORARY PIPES/CULVERTS/RIP-RAP
PADS ETC. IN STREAMS SHALL BE INSTALLED AS OUTLINED IN THE MOST
RECENT EDITION OF THE NC SEDIMENT AND EROSION CONTROL PLANNING
AND DESIGN MANUAL OR THE NCDOT BEST MANAGEMENT PRACTICES FOR
CONSTRUCTION AND MAINTENANCE ACTIVITIES SO AS NOT TO RESTRICT
STREAM FLOW OR CAUSE DIS -EQUILIBRIUM DURING USE OF THIS
CERTIFICATION.
15. ANY RIP -RAP REQUIRED FOR PROPER CULVERT PLACEMENT, STREAM
STABILIZATION, OR RESTORATION OF TEMPORARILY DISTURBED AREAS SHALL
BE RESTRICTED TO THE AREA DIRECTLY IMPACTED BY THE APPROVED
CONSTRUCTION ACTIVITY. ALL RIP -RAP SHALL BE PLACED SUCH THAT THE
ORIGINAL STREAM ELEVATION AND STREAM BANK CONTOURS ARE RESTORED
AND MAINTAINED, NOT RESULT IN THE DE -STABILIZATION OF STREAM BED
OR BANKS UPSTREAM OR DOWNSTREAM OF THE AREA, AND NOT RESTRICT
AQUATIC LIFE PASSAGE.

RIP -RAP SHALL NOT BE INSTALLED IN THE STREAMBED EXCEPT IN SPECIFIC

MAINTENANCE PERFORMED IN A MANNER TO PREVENT CONTAMINATION OF
STREAM WATER FROM FUELS, LUBRICANTS, HYDRAULIC FLUIDS, OR OTHER
TOXIC MATERIALS.
18. HEAVY EQUIPMENT WORKING IN WETLANDS SHALL BE PLACED ON MATS OR
OTHER MEASURES TAKEN TO MINIMIZE SOIL DISTURBANCE.
19, THE CONTRACTOR OR OTHER RESPONSIBLE ENTITY SHALL REPORT ANY
PETROLEUM SPILL OF 25 GALLONS OR MORE; ANY SPILL REGARDLESS OF
AMOUNT THAT CAUSES A SHEEN ON SURFACE WATERS; ANY PETROLEUM
SPILL REGARDLESS OF SIZE THAT OCCURS WITHIN 100 -FEET OF SURFACE
