4a. Describe the existing conditions on the site and the general land use in the vicinity of the project at the time of this application:*
The site is an existing golf course located in the Brier Creek neighborhood of Raleigh, NC. The project area consists of a portion of the cart path where accelerated erosion has
occurred at a stream crossing. The stream headcut has migrated up the drainage to the cart path and is now undermining the cart path bridge. General land use in the vicinity of the
project consists golf course, residential and commercial land.

BCCC was permitted by both the USACE and DWR back in 1998 (WQC 401 Project # 98-1052 / SAW -1999-20084, 85 & 86). Only wetlands existing within this project area back then.
No stream or buffers. The stream has migrated up the drainage into it today. Attached are the 1998 permit approvals and excerpts from the impact maps for this area.

The purpose of the proposed project is to repair the stream, protect upstream wetlands (from draining out of jurisdiction) and protect the existing cart path bridge and large irrigation
line.
4i. Describe the overall project in detail, including indirect impacts and the type of equipment to be used:*
Please refer to attached preliminary construction drawings. The overall project includes the repair of 263 LF of the overall approximately 1,000 LF of tributary stream channel. The
plans all for stabilization of the headcut at and under the existing golf cart bridge to both protect the bridge and large irrigation line. Natural channel design techniques are proposed
(e.g., boulder rock vanes, etc.). The work will also protect upstream wetlands by stabilizing the stream and limiting further migration of the stream channel into the wetlands north of the
bridge which would most likely drain these wetlands. Some grading and wetland restoration work is proposed within 0.0073 acres of wetland area immediately upstream of the bridge as
the channel had begun migrating into this area. Access to the project area will be through upland areas and existing golf course from Clubvalley Way. Earth moving equipment (e.g.,
excavators, dumptrucks, etc.) will be used.

Delineation were verified by the USACE and DWR back in 1998. A site visit was conducted with
Ross Sullivan (USAGE) and Stephanie Goss (DWR) on February 8, 2019 to review the existing
conditions within the project area.

You have our approval, in accordance with the attached conditions, to place fill material in 0.265 acres of
wetlands or waters for the purpose of a residential development, as you described in your application dated
October 21, 1998. After reviewing your application, we have decided that this fill is covered by General Water
Quality Certification Numbers 3101 and 3109. These Certifications allow you to use Nationwide Permit
Numbers 12 and 26 when the Corps of Engineers issues them. This Certification is also covered by Neuse
Buffer Rules (NBR). in addition, you should get any other federal, state or local permits before you go ahead
with your project including (but not limited to) Sediment and Erosion Control, Coastal Stormwater, Non -
Discharge and Water Supply Watershed regulations. Also this approval will expire when the accompanying
404 or CAMA permit expires unless otherwise specified in the General Certification.
This approval is only valid for the purpose and design that you described in your application. If you
change your project, you must notify us and you may be required to send us a new appliclition for a new
certification. If the property is sold, the new owner must be given a copy of the Certification and approval
letter and is thereby responsible for complying with all conditions. If total wetland fills for this project (now or in
the future) exceed one acre, compensatory mitigation may be required as described in '15A NCAC 2H .050$
(h). For this approval to be valid, you must follow the conditions listed in the attached bertifiication.
If you do not accept any of the conditions of this certification, you may ask for an adjudicatory hearing.
You must act within 60 days of the date that you receive this latter. To ask for a hearing, send a written
petition which conforms to Chapter 1508 of the North Carolina General Statutes to the Office of Administrative
Hearings, P.O. Box 27447, Raleigh, N.C. 27611-7447. This certification and its conditions are final and
binding unless you ask for a hearing.
This letter completes the review of the Division of Water Quality under Section 401 of the Clean `.Eater

CONSTRUCTION SEQUENCE The following Construction Sequence applies to the construction of theBrier Creek Golf Course Stream Repair & Stabilization Plan project:1. Obtain USACE, NCDEQ-DWR, and City of Raleigh approval priorto construction.2. Contractor to confirm valid City of Raleigh Land Disturbance,Watercourse Buffer and Floodplain Development Permits (ifrequired) prior to construction.3. Contractor to locate and mark existing utilities and keyinfrastructure on site. Perform site walk-through with Client andEngineer.4. Install temporary construction entrance, silt fence and treeprotection fence as shown on the Construction Drawings.Augment erosion controls at the Engineer's and/or Contractor'sdiscretion during construction.5. Remove trees and under-story vegetation within work area.Where larger trees may be retained Contractor may do so withapproval of the Engineer. Stockpile previously failed trees asappropriate for use during construction.6. Install checkdam immediately downstream of the proposedstream repair reach. Install cofferdam (impervious dike) orEngineer approved equivalent immediately upstream of the startpoint of repair reach. Install temporary splash pad. Install PumpAround Operation. Utilize dewatering pump (if necessary) todischarge to special stilling basin (Dirt Bag).7. Grade stream channel banks in stream restoration reach todimension, pattern and profile shown on the plans and in thedetails. Extent of grading and channel alignment/slope may beadjusted by Engineer at the time of construction. Whereappropriate, blend grade to surrounding topography.NOTE: Contractor shall only disturb length of stream/stream bank that can reasonably be repaired and stabilizedin a single day.8. Backfill eroded areas within upstream wetland area per theConstruction Drawings and as directed by the Engineer at thetime of construction. Blend grade to surrounding topography.9. Install in-channel boulder (and log structures if approved by theEngineer) and constructed riffles as shown on the ConstructionDrawings and as directed by the Engineer.10. Temporarily stockpile cut material fill (if any) in areas as directedby Engineer (or Owner) within adjacent area for later use.Surround stockpile areas with silt fence if directed.11. Apply temporary and permanent seeding measures to disturbedareas as final grade is reached. Apply soil amendments asneeded to prepare for seeding and planting.12. Line restored stream banks with CF-7 matting or Engineer-approved equivalent as construction proceeds. Seed and strawmulch stream banks prior to matting installation. Stabilize otherslopes greater than or equal to 3H:1V with erosion controlmatting.13. Remove downstream gravel checkdam, cofferdam, pump aroundequipment and any other temporary erosion control deviceswhen directed by the Engineer.14. Perform stream bank, buffer and wetland planting as specified.Engineer to be notified prior to planting to approve planting bedconditions.15.
