The North Carolina Division of Water Resources conducted an inspection of the Patriot Place WWTF on
7/19/2019. This inspection was conducted to verify that the facility is operating in compliance with the
conditions and limitations specified in Non -discharge Permit No. WQ0002056. The findings and
comments noted during this inspection are provided in the enclosed copy of the inspection report
entitled "Compliance Inspection Report". There were no significant issues or findings noted during the
inspection and therefore, a response to this inspection report is not required.

Inspection Summary:
A routine compliance inspection of the Patriot Place Mobile Home Park WWTF was conducted on July 19, 2019. The
purpose of the evaluation was to verify that the facility is operating in compliance with the conditions and limitations specified
in Permit WQ0002056. Mike Seely, ORC, Michael Smith, MHP Manager, and Jim Rowan, irrigation and maintenance staff,
were present during the inspection.
This facultative lagoon and irrigation system is permitted for 90,000gpd but only averages approximately 16,000-20,000gpd.
The 3 lagoons are hydraulically connected and the freeboard was 32 inches. There was high vegetation along the berms.
Staff indicated that removal is planned for October. Ownership and name of the MHP changed in spring of 2018 (formally
Gatlin -Ramsey MHP). Operational and maintenance records were organized and the field was well maintained. There are 2
pump stations that are maintained by staff, one pumps flow from the Ramsey MHP, just west of the Patriot Place MHP.
Staff indicated that the pump stations are visited daily and maintenance is documented. Please note that the Permittee is
required to follow all requirements in 15A NCAC 02T .0403 for deemed permitted collection systems.
A review of monitoring reports for the permit cycle shows some frequency violations or missing reports. This was mainly due
to the death of the ORC back in 2016. There have been no GW59s submitted for the 3 monitoring wells since March, 2017.
Staff indicated that the wells have been sampled and analyzed. Please submit the GW59 forms to the Raleigh address
indicated on the permit as soon as possible.
The permit expires on 2/28/2020 and a renewal application is required to be submitted no later that 6 months prior to
expiration. A Operator Designation form needs to be submitted to update the ORC and BORC for this facility.

Comment: Operational and maintenance records were oraanized and comDlete as well as calibration
records for field analyses. EnviroChem, Lab Certification #14, analyze the non -field parameters.
The facility is field lab certified for PH. The annual soil analysis sampling was conducted in the
spring. The monitoring wells have been sampled and analyzed, but the GW59s have not been
submitted since March, 2017. These reports need to be submitted as soon as possible to the
address listed on the permit.

Comment: The field was in good condition and irrigation was observed during the inspection.
There are 2
irrigation pumps and 13 rows of spray heads. Every other row is irrigated at a time
(6-7 rows
total) and then switch to the rows that did not get used for the next spray event. Application
measurment is by pump run times. A reminder that, according to the permit, irrigation equipment
is required to be tested and calibrated at least once per permit cycle.
