penalties in the subject case,received June 3,2019.You have also included your Justification for
Remission Request and have attached documents in support.
However,we do not find attached your Waver of Right to an Administrative Hearing and Stipulation of
Facts(see attached blank form enclosed).Please sign and return this form at your earliest convenience.
The Division must receive your signed document before your request may be processed.
Upon receipt of the above,we will schedule a review.The Director will review your request together with
a combined assessment of$302.10 [penalties plus enforcement costs].Please be advised that the Director
will respond,but may deny, or remit all or part of these penalties in accord with North Carolina General
Statute 143-215.6A(f).
