Dear Permit Holder:
On July 24, 2019, Brandon Wise, inspector with the North Carolina Department of
Environmental Quality (NCDEQ) — Division of Energy, Minerals, and Land Resources, met with Aaron
Grey at the facility located at 3456 Highway 105 South in Boone. The purpose of this visit was to
conduct a compliance inspection as part of NCDEQ's goal to inspect all permitted facilities. The
inspection consisted of review of the Stormwater Pollution Prevention Plan (SWPPP) and each of its
components, review of the facility outfall, as well as the review of the overall site conditions.
Permit
This facility has General Stormwater Permit NCG160039 to discharge stormwater from industrial
activity associated with Asphalt Paving Mixtures and Blocks (SIC 2951] under the National Pollutant
Discharge Elimination System (NPDES). This version of the permit was issued on October 2, 2014 and
the facility has been permitted since March 3, 1995.
Records/Reports:
This facility is required to develop and maintain a SWPPP in accordance with Part II, Section A
of the permit. All qualitative and analytical monitoring records are required to be maintained with the
SWPPP for a minimum of five (5) years. Under this permit, analytical monitoring is required under
current operating procedures. The facility records are kept up to date and kept with the SWPPP in the
main office. Recent monitoring shows that there have been no exceedances and regular sampling is being
performed. The SWPPP for this facility had been put together and implemented prior to the inspection.
The SWPPP was well put together and although the date on the SWPPP was 2015 documentation was
kept in the SWPPP that showed the SWPPP was being reviewed annually. Please note on the site map
that the outfall location needs to be marked. This can be marked manually and does not require a new
map to be created. The facility is required to maintain employee training records in accordance with Part
II, Section A, Item 6 of the permit. Current training records show a single employee being trained in
stormwater, NCDEQ does recommend training all staff, however having the employee responsible for
stormwater is all that is required.
Facility Site Review:
This facility operates as an Asphalt Mixing Plant. The facility is graded in a way that all
stormwater flows to the west of the facility with a berm that goes around the north side of the facility to

redirect stormwater to the west. At the westernmost edge of the facility there is a stormwater pond
designed to contain stormwater from the facility. This pond was full and working during the inspection,
while not required NCDEQ would recommend removing the deposited soil near the influent of the pond.
The outfall on site is located at the discharge from the pond. The overall housekeeping of the facility was
very good especially for an asphalt plant. A storage building was located north of the main office. It
appeared that during heavy rains the potential existed for some stormwater from the site to leave the
facility on the eastern side of this building due to some eroded areas at the rear of the storage building. A
small amount of dirt or other product would be able to fix the eroded area and prevent the potential
stormwater from leaving the site prior to reaching the monitored outfall. A storage area of product was
located on the northern edge of the facility near the main entrance for trucks, this area was concrete and
featured a wall to prevent runoff. It was noted during the inspection some product had been pushed over
the wall and was on the creek side of the wall, this should be removed and monitored to ensure asphalt
product does not run into the surface water. The general housekeeping around the actual plant was very
good, minimal spills and staining were observed.
Effluent Receiving Waters:
Effluent from this facility drains into Laurel Fork, class "C; Tr" waters of the Watauga River
Basin. Please note that this class of water is trout waters and requires lower benchmarks and are subject
to high protection requirements.
Self -Monitoring Program:
This facility is required to provide qualitative monitoring twice annually. The outfall discharge
from the facility is expected to be properly maintained and monitored. At the time of the inspection
monitoring was being done and the outfall looked to be properly maintained. Monthly inspections of the
facility are required to ensure the facility is meeting the housekeeping requirements in the permit. At the
time of the inspection these regular inspections were being completed as required.
There are no additional comments or changes to the currently operating permit. Please continue
to keep up the facility and monitor stormwater discharges. If you have any questions or need additional
